Laurie,
The stream impacts will consist of sediment removal that is generated by the cleaning out of the cross lines at the
designated areas on the provided map. As for the other issue, all of the work for this project will be limited to Greens
Mill Run. The comment on the PCN should have read "See attached map to show the limits of the project ". There is an
overall map included in the submittal that shows areas A -1 that would be disturbed and a individual map is included for
each of those areas explaining what work would be performed at the site. If you have any other questions feel free to
contact me.

'stream name' boxes. To proceed with the application review and approval, please supply this needed information.
Will this project impact any other streams other than Green Mills Run? On page 3 of 11 item 2a, it indicated 'varies with
the project. See attached spreadsheet.' There was no spreadsheet with any of the submitted PCNs.
You may scan in the needed information and email to me or remit hard copies.
