with the permit), all applicable regulations, special conditions and notes set forth below Any violation of these terms may
be subject to fines, imprisonment of civil action, of may cause the permit to be null and void
1) This renewal shall be attached to the original of Permit No 29 -05, which was issued on 2/24/05, as well
as all subsequent renewals, modifications and transfers, and copies of all documents shall be readily
available on site when Division personnel inspect the project for compliance
2) All conditions and stipulations of the active permit iemain in force under this icnewal
This peiinit action may be appealed by the peimittee of
othei qualified pei sons within twenty (20) days of the issuing
date An appeal requues resolution prior to work initiation of
continuance as the case may be
This peimit shall be accessible on -site to Depaitnent
personnel when the project is inspected foi compliance
Any maintenance work or project modification not covered
hereunder requires further Division approval
All work shall cease when the permit expires on
December 31, 2014
In issuing this permit, the State of North Carolina agrees
that your project is consistent with the North Carolina Coastal
Management Program
