REFER TO THE CENTER OF DROP INLETS, MANHOLES AND JUNCTION BOXES, AND THE MIDPOINT OF THE LIP

UTILITIES ARE ILLUSTRATED FOR INFORMATION PURPOSES ONLY. THE CITY WILL NOT BE HELD RESPONSIBLE
FOR THE ACCURACY OF UTILITY LOCATIONS,SIZES, DEPTHS, OR FOR COMPLETENESS OF UTILITY
INFORMATION.
PRIOR TO CONSTRUCTION, THE CONTRACTOR SHALL COORDINATE WITH ALL UTILITY OWNERS WHOSE
FACILITIES WILL BE AFFECTED. THE CONTRACTOR SHALL PROTECT ALL UTILITIES FROM DAMAGE CAUSED BY
HIS OPERATIONS OR THOSE OF HIS AGENTS. THE CONTRACTOR SHALL HOLD THE CITY HARMLESS FOR ANY
THIRD -PARTY INCONVENIENCE CREATED BY WORK OF HIS OWN FORCES OR THAT OF HIS AGENTS. ANY

AS NEEDED, THE CONTRACTOR SHALL ADJUST /RELOCATE THE SANITARY SEWER AND WATER UTILITIES ONLY
ALL OTHER ADJUSTMENTS /RELOCATIONS ARE TO BE PERFORMED BY THE SERVICE PROVIDER(S). THE
CONTRACTOR SHALL COORDINATE ANY RELOCATION WORK WITH THE SERVICE PROVIDER(S) SO AS TO NOT

TREES. SHRUBS. AND HEDGES:
THE CONTRACTOR SHALL REMOVE TREES, SHRUBS, AND HEDGES WITHIN THE LIMITS OF DISTURBANCE UNLESS
SHOWN ON THE PLANS TO BE PROTECTED. WHEN ROOT PRUNING IS NECESSARY, CUT ROOTS CLEANLY
USING A DISC TRENCHER OR OTHER APPROVED METHOD IN ACCORDANCE WITH SECTION 01000 OF THE CITY

TREES SPECIFIED BY THE PLANS TO HAVE ASPHALT CURBING MUST USE CLD STD. 40.13. TREE PROTECTION
SHALL BE IN ACCORDANCE WITH CLD STD. 40.02. WHEN THE TREE IS CLOSE TO THE WORK AREA TREE
PROTECTION CLD STD. 40.12 SHALL BE USED.
CONTRACTOR SHALL OBTAIN APPROVAL FROM THE CITY PRIOR TO REMOVING ANY TREES. IN ADDITION, ALL
TREES LOCATED WITHIN THE LOD THAT ARE TO REMAIN AFTER CONSTRUCTION SHALL BE INSPECTED BY THE
CITY TO VERIFY THEY ARE SUITABLE TO REMAIN.
FOLLOWING CONSTRUCTION STAKING THE PROJECT TEAM (CONTRACTOR, CITY INSPECTOR, CITY ARBORIST AND
CONSULTANT, IF APPLICABLE) WILL HOLD AN ON -SITE MEETING TO ENSURE THE LIMITS OF DISTURBANCE AND
THAT TREES PROPOSED FOR PROTECTION ARE CLEARLY IDENTIFIED. THE TEAM WILL WALK THE PROJECT
AREA AND AGREE ON THE TREES TO BE PROTECTED AND THOSE TO REMOVE, DISCUSS PROPOSED
CONSTRUCTION ACTIVITIES, AND IDENTIFY ANY ADDITIONAL TREES TO PROTECT THAT WERE NOT ORIGINALLY
MARKED AS SUCH. THE CONTRACTOR WILL BE GIVEN APPROVAL FOR TREE REMOVAL, IN WRITING, AFTER
APPROPRIATE TREE PROTECTION HAS BEEN INSTALLED AND BEFORE ANY CLEARING OR REMOVAL CAN BEGIN.
THE CONTRACTOR ASSUMES RESPONSIBILITY FOR "IN- KIND" REPLACEMENT OF TREES DAMAGED THAT HAVE

SIDE SLOPES:
LIMITS OF PROPOSED SLOPES ARE INDICATED IN THE PLANS, DETAILS AND STANDARD DRAWINGS. THE
MAXIMUM SLOPE SHALL NOT EXCEED A 3:1 (HORIZONTAL TO VERTICAL) UNLESS DESIGNATED BY THE
ENGINEER. A CUT SLOPE OF 2:1 MAXIMUM WILL BE USED ONLY AS DIRECTED BY THE ENGINEER.
GRADING
THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL BORROW MATERIAL REQUIRED TO CONSTRUCT THE
PROJECT AS SHOWN ON THE CONTRACT DOCUMENTS.
ALL EXCAVATED MATERIALS THAT ARE NOT REQUIRED OR ARE UNSUITABLE FOR THE PROJECT SHALL BE
CONSIDERED WASTE AND SHALL BE HAULED OFF SITE AND DISPOSED IN A SAFE AND LEGAL MANNER AT THE
CONTRACTOR'S EXPENSE.
CONTRACTOR SHALL FOLLOW THE FILL PLACEMENT PROCEDURES SHOWN IN SP -02 OF THE PROJECT SPECIAL

REPORT NORTHPARK POND DAM (S &ME PROJECT NUMBER: 1351 -12 -039, DATED APRIL 24, 2012). IN
ADDITION, THE CONTRACTOR SHALL MAKE HIS /HER OWN INVESTIGATION AS TO THE SUBSURFACE CONDITIONS.

MAILBOXES:
THE CONTRACTOR SHALL RELOCATE ALL MAIL BOXES AS REQUIRED BY SECTION 107 -12 OF THE NCDOT
STANDARD SPECIFICATIONS FOR ROADS AND STRUCTURES. COORDINATE THIS WORK WITH THE U.S. POSTAL

FENCES:
THE CONTRACTOR SHALL REMOVE AND RESET FENCES AS NOTED ON THE PLANS AND /OR AS DIRECTED
BY THE ENGINEER.
PAVEMENT DEGRADATION FEE:
CDOT HAS DETERMINED THAT NO PAVEMENT DEGRADATION FEE IS TO BE ASSESSED.
TRAFFIC CONTROL:
TRAFFIC CONTROL SHALL BE IN ACCORDANCE WITH THE 2006 EDITION OF THE "WORK AREA TRAFFIC CONTROL
HANDBOOK" AND PER SHEET 13.
STORM DRAINAGE PIPE & GRADING NOTES:
PROJECT TOPOGRAPHY AND EXISTING CONDITIONS GROUND - CONTROLLED SURVEY PROVIDED BY ESP

AND SEDIMENT CONTROL INSPECTOR 48 HOURS PRIOR TO ANY LAND DISTURBING ACTIVITY IS A VIOLATION OF
CHAPTER 18 OF THE CITY CODE AND IS SUBJECT TO A FINE.
8. INSTALL PERMANENT VEGETATIVE COVER AND THE LONG -TERM EROSION PROTECTION MEASURES OR STRUCTURES
AS SOON AS PRACTICAL IN THE CONSTRUCTION PROCESS. APPROPRIATE EROSION CONTROL MEASURES MUST BE
PLACED BETWEEN THE DISTURBED AREA AND AFFECTED WATERWAY AND MAINTAINED UNTIL PERMANENTLY
VEGETATED.
9. ALL AREAS DISTURBED BY THE CONTRACTOR SHALL BE SEEDED PER THE SPECIFICATIONS SHOWN IN DETAIL 5/11
OR AS OTHERWISE DIRECTED BY THE ENGINEER. AREAS BELOW ELEVATION 765.50 WITHIN THE HISTORIC POND
LIMITS, AS SHOWN ON SHEET 3, SHALL NOT BE SEEDED. STABILIZATION MEASURES SHALL BE INITIATED AT THE
END OF EACH DAY IN PORTIONS OF THE SITE WHERE CONSTRUCTION ACTIVITIES HAVE TEMPORARILY OR
PERMANENTLY CEASED EXCEPT WHERE CONSTRUCTION ACTIVITIES SHALL BE REINITIATED WITHIN 21 CALENDAR
DAYS.
10. USE SPECIFIED EROSION CONTROL MATTING AS SHOWN ON PLANS PER DETAIL 5/12. APPLY 3000 LB /ACRE
GRAIN STRAW AND ANCHOR STRAW ON ALL OTHER DISTURBED AREAS. ASPHALT TACKIFIER SHALL NOT BE USED
FOR ANCHORING STRAW.
11. CONTRACTOR SHALL PERFORM GRADING OPERATIONS IN A MANNER AND SEQUENCE TO MINIMIZE THE DISTURBANCE
OF SURFACE COVER. THE CONTRACTOR SHALL NOT DISTURB MORE AREA THAN CAN BE STABILIZED IN ONE
WORKING DAY.
12. EROSION CONTROL MEASURES MAY BE PHASED -IN TO THOSE AREAS OF THE PROJECT CURRENTLY BEING WORKED
ON. THE CONTRACTOR MAY MODIFY OR RELOCATE EROSION CONTROL MEASURES TO MAKE ADJUSTMENTS FOR
UNFORESEEN FIELD CONDITIONS SO LONG AS PROPER CONSTRUCTION IS MAINTAINED TO ENSURE THE INTEGRITY
AND USEFULNESS OF THE PROPOSED MEASURES. ALL RELOCATION SHALL HAVE THE APPROVAL OF THE
ENGINEER. ALL DISTURBED AREAS ALONG CHANNEL BANKS SHALL BE STABILIZED WITH TEMPORARY SEED AND
MULCH AT THE END OF EACH DAY.
13. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTAINING ALL EROSION CONTROL MEASURES RELATED TO THE
CONSTRUCTION SITE DURING ALL PHASES OF CONSTRUCTION. EROSION CONTROL MEASURES SHALL BE PROPERLY
MAINTAINED UNTIL PERMANENT VEGETATION IS ESTABLISHED.
14. ALL SEDIMENT AND EROSION CONTROLS ARE TO BE INSPECTED AT LEAST ONCE EVERY SEVEN CALENDAR DAYS OR
AT THE END OF EACH WORKING DAY AND AFTER ANY STORM EVENT OF GREATER THAN 0.5 INCHES OF
PRECIPITATION DURING ANY 24 -HOUR PERIOD. MAINTENANCE OF SEDIMENT TRAPPING STRUCTURES SHALL BE
PERFORMED AS NECESSARY PER THESE INSPECTIONS. TRAPPED SEDIMENT SHALL BE PROPERLY REMOVED FROM
THE UPSLOPE SIDE OF SEDIMENT FENCE WHEN SEDIMENT BUILDS UP TO A DEPTH OF 0.5 FEET. SEDIMENT FENCE
SHALL BE REPAIRED AS NECESSARY TO MAINTAIN ITS FUNCTION.
15. THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE IMMEDIATE REMOVAL OF ANY SOIL TRACKED INTO THE PUBLIC
RIGHT -OF -WAY. THE CONTRACTOR MUST TAKE THE NECESSARY ACTION TO MINIMIZE THE TRACKING OF MUD
ONTO THE PAVED ROADWAY FROM CONSTRUCTION AREAS. DAILY REMOVAL OF MUD /SOIL MAY BE REQUIRED.
POWER WASHING OF SOILS INTO THE DRAINAGE SYSTEM IS PROHIBITED.
16. THE LOCATIONS OF SOME EROSION CONTROL MEASURES MAY HAVE TO BE ALTERED FROM THOSE SHOWN ON THE
PLANS IF DRAINAGE PATTERNS CHANGE DURING CONSTRUCTION.
17. CONTRACTOR SHALL PROVIDE FOR HANDLING THE INCREASED RUNOFF CAUSED BY CHANGED SOIL AND SURFACE
CONDITIONS. USE EFFECTIVE MEANS TO CONSERVE EXISTING ON -SITE SOIL INCLUDING THE USE OF DIVERSION
DITCHES.
18. IF IT IS DETERMINED DURING THE COURSE OF CONSTRUCTION THAT SIGNIFICANT SEDIMENT IS LEAVING THE SITE,
THE CONTRACTOR IS OBLIGATED TO TAKE ADDITIONAL PROTECTIVE ACTION.
19. ALL EXCAVATED MATERIAL MUST BE PLACED AT A MINIMUM OF 5 FT FROM THE EXISTING STREAM. SILT FENCES
MUST BE INSTALLED AROUND ALL STOCKPILE AREAS.
20. WHERE WORK IS BEING COMPLETED INSIDE THE POND, THE DAM AND INSIDE THE EXISTING STREAM CHANNEL,
TEMPORARY COFFER DAMS, IMPERVIOUS DIKES, AND BYPASS PUMPING WILL BE USED TO DE -WATER THE WORK
AREA.
21. CONTRACTOR SHALL NOT COMPACT SOIL AROUND ROOTS OF TREES TO REMAIN, AND SHALL NOT DAMAGE SUCH
TREES IN ANY WAY.
22. IT SHALL BE UNDERSTOOD THAT FAILURE TO SPECIFICALLY MENTION ANY WORK THAT WOULD NATURALLY BE
REQUIRED TO COMPLETE THIS PROJECT SHALL NOT RELIEVE THE CONTRACTOR OF HIS RESPONSIBILITY TO
PERFORM SUCH WORK.
23. ALL NECESSARY MEASURES MUST BE TAKEN TO PREVENT OIL, TAR, TRASH, AND OTHER POLLUTANTS FROM
ENTERING THE ADJACENT OFFSITE AREAS.
24. CONSTRUCTION ACTIVITIES MUST AVOID TO THE GREATEST EXTENT PRACTICABLE, ENCROACHMENT INTO ANY
WETLAND /STREAM AREAS NOT DESIGNATED AS IMPACT AREAS.
25. NO ON -SITE BURIAL OF VEGETATION OR CONSTRUCTION DEBRIS WILL BE PERMITTED.
26. ANY GRADING BEYOND THE LIMITS OF DISTURBANCE SHOWN ON THE PLAN IS A VIOLATION OF THE NORTH
CAROLINA EROSION CONTROL ORDINANCE, AND IS SUBJECT TO A FINE.
27. PLEASE REFERENCE PLAN SHEET DETAILS AND NCDENR STANDARDS FOR CONSTRUCTION OF EROSION CONTROL
MEASURES.
28. UPON COMPLETION OF FINAL GRADING, PERMANENT VEGETATION SHALL BE ESTABLISHED WITHIN 15 WORKING DAYS
OR 90 CALENDAR DAYS (WHICHEVER IS SHORTER). SEEDING SHALL BE IN ACCORDANCE WITH DETAILS 5A, B, C,
& D ON SHEET 11.

W) CONTRACTOR SHALL NOT BE ALLOWED TO WORK ON BOTH SIDES OF THE ROAD SIMULTANEOUSLY WITHIN
THE SAME AREA EXCEPT WHERE THE ROADWAY IS DIVIDED BY A RAISED MEDIAN. IT WILL BE ACCEPTABLE TO
CONSTRUCT BORE PITS ON EACH SIDE OF A ROADWAY FOR BORING UTILITIES UNDER THE ROADWAY UNLESS
SUPERCEDED BY ENGINEER TO COMPLY WITH PEDESTRIAN REQUIREMENTS OR LATERAL CLEARANCEFROM
TRAVEL LANES.
