This office has reviewed the subject erosion and sedimentation control plan sealed by Kevin
Varnell, PE. We find the plan to be acceptable and hereby issue this letter of approval. This letter
of approval shall be valid for a period not to exceed twenty-four months or the time required for
the proposed construction activities, whichever comes first.
Please be advised that Section 801, Sub -section E12 of the City of Rocky Mount's Land
Development Code (hereinafter referred to as LDC) requires that a copy of the approved erosion
control plan be on file at the job site. Also, you should consider this letter the notice required by
the LDC, Section 802, Sub -section Q 1 of our right of periodic inspection to ensure compliance
with the approved plan.
The City of Rocky Mount's Sedimentation Pollution Control Program is performance oriented,
requiring protection of the natural resources and adjoining properties. If following the
commencement of this project, it is determined that the erosion and sedimentation control plan is
inadequate to meet the requirements of the Soil Erosion and Sedimentation Control Ordinance of
the City of Rocky Mount (Section 801 of the LDC, also NCGS 113A-51 through 66,) this office will
require revisions to the plan and implementation of the revisions to ensure compliance with the

Ordinance. Performance Reservations for this plan are included as part of this approval. The
developer is responsible for the control of sediment on -site. If the approved erosion and
sedimentation control measures prove insufficient, the developer must take additional steps
necessary to stop sediment from leaving this site. Each sediment storage device must be inspected
after each storm event. Maintenance and/or clean out, is necessary anytime the device is at 50%
capacity.
Please be aware that this approval is based in part on the accuracy of the information in the
Financial Responsibility/Ownership Form, which you have provided. You are required to file an
amended form if there is any change in the information included on the form. Also, please note
that this approval does not relieve the permittee of the responsibility to obtain any other Federal,
State, or Local approvals or permits. The responsible party may need to contact the appropriate
Federal, State, and Local agencies prior to beginning work. This approval does not permit the
construction of any impervious surface, nor does it allow for the placement of fill in any area for
which a floodplain development permit is required.
In addition, LDC Section 801-E10 requires that you notify this office of the proposed starting date
for this project. Your cooperation is appreciated, and we look forward to working with you on this
project.
Also, this letter gives the notice required by G.S. I I3A-61.1(a) of our right of periodic inspection
to insure compliance with the approved plan.
As of April 1, 2019, all new construction activities are required to complete and submit an
electronic Notice of Intent (NOI) form requesting a Certificate of Coverage (COC) under the
NCGO10000 Construction Stormwater General Permit. This form MUST be submitted prior to
the commencement of any land disturbing activity on the above project. The NOI form may be
accessed at dgg.nc.&ov/NCG01. Please direct questions about the NOI form to Annette Lucas at
Annette.lucas(&ncdenr.gov or Paul Clark at Paul.clark(@.ncdenr.gov.
After you submit a complete and correct NOI Form, a COC will be emailed to you within three
business days. A $100 fee will be charged annually. This fee must be sent to the DEMR
Stormwater Central Office staff in Raleigh.
Title 15A NCAC 413 .0118(a) and the NCGO I permit require that the following documentation
be kept on file at the job site:
1. The approved E&SC plan as well as any approved deviation.
2. The NCGO 1 permit and the COC, once it is received.
3. Records of inspections made during the previous 30 days.
