It shall be responsibility of City of Conover to ensure that the as -constructed project meets the
appropriate design criteria and rules. Failure to comply may result in penalties in accordance with the
North Carolina General Statute §143-2I56A through §143-2I6.6[' construction of additional or
replacement wastewater collection facilities, and/or referral of the North [ono|ina'licensed Professional
Engineer tmthe licensing board.

In accordance with the provisions ofArticle 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations, permission is hereby granted to the
City of Conover
Catawba County
for the construction and operation of approximately 480 linear feet of 8-inch force main sewer and the
discharge of 0 gallons per day of collected domestic wastewater into the City of Conover Northeast
WWTP existing sewerage system, pursuant to the application received August 23, 2013, and in
conformity with 1.5A NCAC 2T; the Division's Gravity Sewer Minimum Design Criteria adopted February
12, 1996 as applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump
Stations and Force Mains adopted June 1, 2000 as applicable; and other supporting data subsequently
filed and approved by the Department of Environment and Natural Resources and considered a part of
this permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the

meet I5A NCAC 2T; the Division of Water Resources' (Division) Gravity Sewer Minimum Design Criteria
adopted February 12, l996 as applicable,- and the Division's K4(nimUm Design Criteria for the Fast -Track
Permitting of' PumpStations arid Force Mains adopted June 1' Z0OOasapplicable, unless specifically
mentioned herein. Division approval is based on acceptance of the certification provided by a North
Carolina -licensed Professional Engineer inthe application. it shall be the 9ermittee'sresponsibility to
ensure that the as -constructed project meets the appropriate design criteria and rules.

INSTRUCTIONS TO TH E APPLICANT:
A. Attachment Form:
If the sewer system project area is a minimum of 100 feet from any surface water or wetlands, this
classification is NOT necessary. If any portion of the sewer system project is within 100 feet of any
surface water or wetlands, this form must be completed if there are proposed design conflicts with
setbacks or buffers as listed in 2T 0305 (f)..
• Do not submit this attachment form for review without a corresponding permit application (Form
FTA -03/2012) unless requested by the Division,
Any changes to this attachment form will result in the application package being returned,
B. Prepare the attachment form with the requested information for each portion or location of
the sewer system that is in conflict with a waterbody or wetlands.

document is available from our web site at the address shown above or by contacting the
appropriate Division of Water Quality regional office,
The same Professional Engineer who certified the permit application form should seal this form,
Different Professional Engineering seals may be accepted from engineers within the same firm,
Submit an 8,5-inch by 11-inch COLOR copy of a USGS Topographic Map of sufficient scale to
identify the entire project area and the closest surface waters. Each map or maps must show the
location of the sewer line and pump stations and be of reproducible quality. For instance, the
project involves 4 miles of interceptor sewer that traverses over or near several different
waterbodies (or counties, basins, etc.). The map should have location ID's for each different
waterbody (where the sewer line is within the setback/buffer) and corresponding classifications
should be recorded.
C. Include the attachment form and the map portions with the permit application for submittal to
the appropriate regional office.

We are submitting,. on the behalf of the City of Conover, the Fast -Track Application for the referenced
project. Enclosed are two copies of the executed application form, color vicinity map, Watershed
Classification Attachment, calculations, and application fee of $480.
On July 27, 2013 torrential rainfall resulted in excessive flooding across Catawba County, Lyle Creek
rose to record levels causing the failure of the Ridge Road NE Bridge in the Rock Barn community. The
damage to the bridge is not repairable and will require complete replacement Three public utilities
run through the bridge structure, including: an 8water line, an 8' gravity sewer line, and an 8' sewer
force main, These three utilities are currently intact and being suspended by what is left of the bridge.
Further erosion of the bridge sub -grade will cause the ultimate 'failure of the utilities. The existing
water line has been taken out of service with the closure of main line valves. Plans for temporary
measures are in place for the gravity sewer line, In lieu of providing temporary measures for the
sewer force main,, the OWNER wishes to permanently relocate the force main to cross under Lyle
Creek. The proposed force main crossing of Lyle Creek will be made by horizontal directional drilling
methods. This will minimize environmental impacts and disturbance to the surrounding golf course,
As previously discussed, the enclosed permit application precedes the final completion of plan and
specifications. Upon your request we will provide final design drawings. They should he complete
within a few days. The projected is scheduled for bidding and construction, as soon as possible.
The proposed force main is a relocation or an existing line and there is no additional wastewater ow
being permitted. Therefore, the Flow Tracking/Acceptance Form is not required. Per your
recommendation the force main relocation is being permitted as a modification to the original permit
number WQ0020314.
