Everything looks good except for one small issue: Please correct the following: - Please provide an original signed O&M Agreement Form (The cover page of the O&M Agreement Form was modified to address previous comments. Since the original hard copy of the cover page of this form was notarized, it must be re-signed and re-notarize since it has been changed). Please provide the following: - 1x signed & notarized original hard copy of the O&M Agreement Form. - 1x electronic copy of the signed & notarized O&M Agreement Form (if possible). Hard copies should be mailed to me at the following address: 512 N. Salisbury Street 1612 Mail Service Center Raleigh, NC 27699-1612 Electronic copies should be uploaded at the following location: https://edocs.deq.nc.gov/Forms/SW_Project_Submittal Since this is a really minor thing, once I get the hard copies, anything else that may have changed from addressing these issues, and the documents are uploaded to Laserfiche (Provided everything is still in order), we can issue the permit without doing a formal 30-45 day review. A review of that information has determined that the application is not complete. Attached is a letter describing the items that need to be submitted.
