Dear Mr. Srivastava,
On April 14, 2021, staff of the North Carolina Division of Water Resources Winston-Salem
Regional Office (DWR) performed a follow-up inspection of the subject wastewater irrigation
system. This inspection was conducted by DWR staff person Patrick Mitchell and focused on
follow-up on noncompliant items previously noted for the subject facility. The following violations
were found to persist and therefore warrant your attention and action:
1. The three valves serving the activated sludge return continue to be in disrepair. One of
the three valves had been removed completely. It should be noted that this was included
in a Notice of Deficiency dated November 26, 2018 and has yet to be properly addressed.
Failure to maintain the proper operation of these valves is impacting the wasting of sludge
from the system. Please repair and/or replace these valves as soon as possible and
notify this office when completed.
2. The staff gauge serving the 500,000 gallon effluent storage tank is not operating properly.
A properly functioning staff gauge is important for monitoring and maintaining adequate
freeboard in the storage tank. This item was also noted in the November 26, 2018 Notice
of Deficiency and has yet to be properly addressed. Please have this staff gauge repaired
or replaced as soon as possible and notify this office when completed.
To avoid further enforcement actions, please address the violations described above as soon as
possible and notify this office in writing when completed. This office request that you submit a
written response within 30 days of receipt of this Notice. Please include in this response an outline
for actions taken to remedy these items. If you wish to present an explanation for the violations
cited, or if you believe there are other factors which should be considered, please include such

Inspection Summary:
On April 14, 2021, WSRO staff conducted a follow-up reconnaissance inspection of the subject facility. The primary purpose
of this inspection was to follow up on deficiencies noted in the November 26, 2018 NOD and in the February 5, 2019
follow-up inspection. The inspection found that two of the items noted in the 2018 NOD and in the 2019 follow-up inspection
are yet to be properly addressed. The violations listed below require attention and action.
1. The three valves serving the activated sludge return are still in need of repair or replacing. The ORC indicated they have
had issues with finding a welder to make the repairs. Request the valves be repaired/replaced as soon as possible and that
WSRO be notified when completed.
2. The staff gauge on the 500,000 gallon effluent storage tank is still not working properly. Request that the gauge be
repaired or replaced as soon as possible and that WSRO be notified when completed.
* This inspection focused only on review of items requiring follow up from previous inspections and was not a comprehensive
inspection.
A comprehensive compliance inspection will be conducted within the next 12 months.
