Pam,
As we discussed I've attached an Agent Authorization form, a sketch template, and the application for the 401 (which it sounds
like you already have). You'll need to submit 5 copies of the application, along with the $240 fee to the address on the
application (that is still correct). Your owner should sign the application — then through the Agent Authorization he can delegate
the contractor or you to act on his behalf.
We'd also like a few digital photos (w /scale) to demonstrate the amount of erosion (this is helpful for me in case I do not get an
opportunity to visit the property — and it also provides demonstration of the need for the project).
As we discussed — both NCDWQ and Duke Energy lake services pushes folks to use rip /rap in areas that are not so eroded, then
move to boulder /native stone wall (w /rip -rap) in areas of higher erosion (over appx. 3 ft of erosion).
If you'll also have your contract show the length of shoreline that will be rip -rap, and the length that will be boulder /rip -rap —
that will be most helpful.
I believe Lisa Leatherman — copied above — requires a site vl t prior to you folks submitting an application to us (and after
DWQ's approval is completed — she can do Duke Energy'roval, and then work may proceed).

The following is an abbreviated application form for those applying for a 401 Water Quality
Certification to impact FERC (Federal Energy Regulatory Commission) regulated lake shore or
bottom under the U.S. Army Corps of Engineers Permit GP30. This Application cannot be used
for wetland or stream impacts. Please provide five (5) copies of this application and supporting

Directions to 926 Strawberry Hill Road Cullowhee, NC
1.) Begin at the intersection of highway 107 and 64 in Cashiers. There will be
an Exxon station on the corner to your left.
2.) Turn left onto highway 107 here. You will be traveling North on 107 for
about 15 miles. You pass Happ's Restaurant and then JJ Cantina/ service
station /convenience store both on your right. Look for Pine Creek Road on
the left. There is a boat storage facility at the intersection. Turn left on Pine
Creek.
3.) Continue on Pine Creek (Traveling West) you will cross the dam and
Lake Glenville will be on your left. Continue on the "long and winding
road that leads to our door." You will pass a pasture on your right where
the "Welfare Donkeys" live and you will see a sign that says
FRANKLIN 4 DO NOT go toward Franklin but veer to the left. Travel on
until you pass a development on your left called "Edgecliff." Just past
Edgecliff look for green street sign that says Strawberry Hill Road- Turn
left here. After you turn onto Strawberry Hill Road DO NOT follow Cherry
Hill Lane to your right. Continue straight until you reach 926 Strawberry
Hill Road on your right. Veer to the right onto our driveway. Welcomel
NOTE: At the left turn at Strawberry Hill Rd. and the right turn into our driveway, you
will not be able to see the road over the hood of your car as the turn is steep. After
you have whispered a prayer and are trusting in God- Make the turn and you will see
the road. As long as there are no oncoming cars just go ahead and turn, everything
