Box 370 Seagrove, North Carolina 27341 SUBJECT: Follow Up Inspection Seagrove — Ulah Metropolitan Water District WWTF Permit No. WQ0002648 Randolph County Dear Mr. Walker: On March 29, 2021 Division of Water Resources staff person Caitlin Caudle conducted a scheduled follow up inspection of the facility. Larry Chilton, ORC, and Russell Welch, Back Up ORC, were present during the entire inspection. The following items were discussed during the follow up inspection, and may require additional actions: • The driveway leading to the sludge basin was repaired, with additional efforts being made to divert water upslope to prevent further erosion. This was listed as an item of concern in correspondence dated August 4, 2020. • Permit Condition 1.1 has been completed. All pipes leading to the industrial treatment system have been covered, with only influent pipes open. • Permit Condition 1.2 has been completed. All pipes not associated with the domestic treatment system have been capped. • Permit Condition 1.3 has been completed. Mr. Chilton provided documentation stating that the new synthetically lined lagoon has been connected to the old storage pond since installation. • The ORCs stated that there is some confusion related to transferring effluent from the existing storage pond to the new equalization basin (Lagoon #2). The ORCs have not been provided guidance or a start up procedure for transferring effluent between the storage pond and Lagoon #2. Please acquire a start up procedure or any additional guidance from the engineer to safely transfer effluent between the storage pond and Lagoon #2. The following permit conditions have yet to be completed and require further actions: A. Per Permit Condition 1.4, an engineer certification must be provided certifying that the permitted facility has been constructed and the disconnections implemented according to G.S. 143-215.1, Administrative Code Title 15A Subchapter 2T, the subject permit, and the Division -approved plans and specifications. After reviewing information submitted during the permit renewal process, the as -built plans and description of the facility are not representative of what is installed. Please submit an engineer certification as soon as possible, with a detailed description of the system components that differ from the as -built plans. B. Per Permit Condition 1.6, two original copies and one digital copy of a site map shall be submitted. Please review the subject permit for all items that must be included in the site map. Please submit the required site maps as soon as possible. C. Per Permit Condition 1.7, within 90 days of the effective date of the permit, a revised Operation and Maintenance Plan shall be submitted to the Division for review. To date, no Operation and Maintenance Plan has been submitted to the Division. The ORC and BUORC stated that they provided all necessary information for the engineer to create an updated plan. Please submit an Operation and Maintenance Plan as soon as possible. D. Per Permit Condition 1.8, within 14 months of permit issuance a 12-month study of the influent flow at the facility shall be conducted to assess the design capacity for obligated sewer connections. Please refer to the permit for the necessary information to include in the flow study. It should be noted that a portion of the months included in the flow study may experience uncharacteristically low flows due to decreased tourist activities in the Seagrove — Ulah area, and decreased activity at schools and businesses. The data collected during the study period may not accurately represent business -as - usual flows. Proposed sewer connections, such as listed below, should be accounted for as a potential source of influent. E. Expansions of the current sewer collection system could result in increased flows at the wastewater treatment facility. It has come to the attention of this Office that a development project has requested to connect to the Seagrove — Ulah sewer collection system, with the designed flow for this development being 11,400 gallons per day (GPD). The current permitted flow for this facility is 30,000 GPD. Any exceedances of this monthly average will result in a violation of the subject permit, and continued violations of this nature could result in further actions from the Division. Please be mindful of the potential for violations resulting from exceedances of the permitted flow and be prepared to provide this Office with a plan of action for how this additional flow will be managed. Please refer to the enclosed inspection report for additional details from the follow up inspection. Schedule Condition 1.1: Eliminate all piping stemming from the splitter box at the head of the old industrial treatment system within 30 days. The facility has plugged the pipes leading to the other parts of the treatment plant. The only pipes remaining open are the domestic and industrial influent pipes. Schedule Condition 1.2: Seal or eliminate all piping to the sludge storage basin that are not connected to the domestic wastewater treatment system or the decant valve within 30 days. The facility has capped the pipes from the industrial treatment system. The only pipes left uncapped are the decant valve from the digestor, and a vertical pipe leading to the storage pond that serves as an emergency overflow for the sludge basin. Schedule Condition 1.3: Install a transfer piping connecting the treated effluent in the 588,000 gallon storage pond to the pump station and to the 2,005,265 gallon synthetically lined lagoon (i.e. Lagoon #2) within 60 days. According to the ORCs, the transfer piping has been installed since the new lagoon was installed. The ORCs stated that the previous ORC did not share a change in the construction plans with the current ORCs while the lagoon was being built. As a result, the current ORCs were not aware that the transfer piping had been altered and installed. After locating the piping from Lagoon #2 the ORCs found plans that illustrated how Lagoon #2 was connected. The ORCs provided this documentation to illustrate that the piping was installed. The ORCS have not put Lagoon #2 into use as there is some confusion about the valves needed to change the flow of effluent. The ORCs are waiting on startup plans or guidance from the engineer. Schedule Condition 1.4: After completing 1.1, 1.2, and 1.3 the facility shall submit an engineering certification. The ORCs were unsure about the requirement to submit an engineering certification as there was no construction completed to connect Lagoon #2. Schedule Condition 1.5: After completing 1.1, 1.2 and 1.3 the facility shall notify WSRO so that the Division can conduct an inspection and ensure compliance with all applicable rules and regulations. The ORC did notify Ms. Caudle of progress with completing the permit conditions. Schedule Condition 1.6: Engineer certification requirement. To date, an engineer certification has not been received. Schedule Condition 1.1: Revised O&M plan within 90 days. The ORCs provided the Board of Directors with an updated day to day operations plan, which was reportedly passed along to the engineer. The ORCs were reminded that a flow study must be completed within 14 months of the effective date of the permit. The facility is also reminded that any upgrades or expansions to the existing wastewater treatment system must be submitted to the Division as a permit modification request. Other Observations: • The gravel road leading to the sludge basin has been repaired, and measures taken to divert the stormwater flow. • The irrigation system was calibrated before the end of the permit cycle. • The gutter pipe of the chlorinator building was piped directly into the chlorinator. Ms. Caudle asked that this pipe be removed. The ORCs mentioned that other stormwater may be piped into the system. Two irrigation zones are offline with repairs planned. Review of Files 3/31/2021 On March 31, 2021 Ms. Caudle completed a review of the WSRO electronic files. In this review, the 2019 recertification of the facility describes the transfer process between the smaller storage pond and the larger storage pond. As described in the additional information submitted during permit renewal: "There are two pump stations with intakes in the 0.58MG pond. The original older pump station with two 75 HP pumps applied water o the spray fields for land application, while the second newer pump station with two 75 HP pumps, constructed in 2007, but soon to be completed, shall move water from the 0.58 MG pond to either the spray fields or to a 2007 constructed 2 MG treated water equalization lagoon if needed. Water pumped to the 2 MG lagoon can then fall back by gravity to the 0.58 MG pond for land application when conditions permit." Another note is listed on a diagram of the proposed facility modification when the new lagoon was installed. The note is describing the connection between the two lagoons. The note is as follows: "For unknown reasons, this section of pipe was never installed to completely attach the 2.0 MG lagoon to the existing system. Completion of the attachment and certification shall be completed by August 2020." The as -built plans provided with this information are incorrect.
