In accordance with the request for discharge permit application received on July 24, 1998, the
Division is forwarding herewith the subject state - NPDES permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated December 6, 1983.
The following modification has been made to the final permit: the requirement for upstream
and downstream monitoring for fecal coliform and conductivity has been deleted. All other limitations
and monitoring requirements remain unchanged. -
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the office of Administrative Hearings,
Post Office Drawer 27447, Raleigh,North Carolina 27611-7447. Unless such a demand is made,this permit
shall be final and binding.
Please take notice that this permit is not transferable. Part fI, EA. addresses the requirements to be
followed in case of change in ownership or control of this discharge. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, Coastal Area Management Act, or any other Federal or Local
governmental permits may be required.

1. Continue to operate an existing 0.080 MGD wastewater treatment system
consisting of an influent pump station, bar screens, two aeration basins, two
secondary clarifiers, gravity sand filters, chlorine contact chamber and effluent
flow measurement. The facility is located at Willow Creek Builders Wastewater
Treatment plant, southwest of Willow Creek County Club, High Point in
Davidson County, and
2. Discharge from said treatment works at the location specified on the attached map
into Abbotts Creek, which is classified WS-III waters in the Yadkin-Pee Dee
River Basin.

Review of compliance data indicated that facility appears to be well operated. It has met
all the limits.
Per Yadkin Basin Plan -Recommended TMDLs and Management Strategies for Oxygen
consuming wastes for the Abbotts Creek watershed- No new dischargers of oxygen
consuming waste. Thomasville and Lexington should serve, as regional WWTPs for
future wastewater needs.
An ambient station on Abbotts Creek at State Road 1243 at Lexington has recorded fecal
coliform levels with geometric mean exceeding 200 colonies/100ml. A value of
659/100ml was detected at this station. It is believed the fecal problems are primarily of
a nonpoint source origin. Exceedances occur where there are no significant upstream
dischargers and most facilities are in compliance with permit limits.
Recommend renewal of the permit with all existing limits. All parameters and
monitoring frequencies remain the same, with the exception of the monitoring
frequency for TN and TP being increased to monthly from quarterly.

RESPONSE:
This agency has reviewed the draft permit and determined that the proposed
discharge will not be sufficiently close to any existing or know proposed
public water supply intake so as to create an adverse effect on water quality.
We concur with the issuance of this permit provided the facility is operated
and maintained properly, the stated effluent limits are met prior to discharge,
and the discharge does not contravene the designated water quality standards.
