Division personnel have reviewed and approved your application. Accordingly, we are forwarding herewith the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated December 6, 1983, and as subsequently amended. The following changes have been made to the subject permit: • The pH monitoring requirement has been removed. • Monitoring requirements for Iron, Chloride and Total Residual Chlorine have been added. • Monitoring frequencies for all parameters have been added. • '"'reekly- instream turbidity monitoring has been added. Quarterly Total Phosphorus, Total Kjeldahl Nitrogen and Nitrate/Nitrite monitoring requirements have been removed from the subject permit. Sampling for these parameters may be halted immediately. The documented results from the monitoring data submitted to the Division are well below the established levels of concern (3.0 mg/l for Total Nitrogen and 1.0 mg/1 for Total Phosphorus). Please note that a separate discharge permit (NC0086291) is being issued for the Beaver Hill Water Treatment Plant. The subject permit applies to the Freemason Water Treatment Plant only. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 2 7447, Raleigh, North Carolina 27611-744 7. Other Special Items: none PART IV - EVALUATION AND RECOMMENDATIONS The results of the inspection indicate that the plant was in compliance with the limitations and conditions of the permit. In general, the plants are well maintained. The effluent from both plants appeared clear and free of visible solids. The biota of Filbert Creek did not seem to be affected by the discharges. A cursory review of data generation and tracking, sampling practices, and self-monitoring data revealed no discrepancies. It is recommended that the permit be renewed for a period of five years. He called back- discharge is within 1000'; he states that in these cases, RO leaves it as one permit. He will call back with permit number if he thinks of another permit with the same condition. 1/26 Left a message with Roger Thorpe about whether to issue 2 permits or not. 1/29 Roger Thorpe is OK with issuing 2 permits. 2/3 New permit# for Beaver Hill WTP is NC0086291.
