(stream 1) located at Station 14 +36 was originally permitted for 4 feet of permanent stream impacts as NCDOT was planning
to replace an existing 36 foot long pipe with a 40 foot long pipe On December 4, 2012, 1 performed an inspection of the site
and measured the pipe that had been installed at Station 14 +36 and found that the pipe plus headwalls on both ends
combination measured a length of 44 feet The original plan sheets did not call for headwalls on the subject pipe In talking
with the District Office, it was found that in scoping the road for construction, personnel determined that a 40 feet length of
pipe was needed at the subject location It was also noted that headwalls were needed during that scoping However,
headwalls were inadvertantly not shown on the plan sheets Also, the accounting for the 2 foot length of stub on each
headwall used to connect the headwall to the pipe was inadvertantly not accounted for Also, this unintentional oversight with
regard to the extra cumulative four feet of stub carried over throughout the construction process resulting in the cumulative 4
feet of additional permanent stream impacts at Station 14 +36 beyond what had been permitted For future projects NCDOT
plans to have all internal parties involved including construction personnel scoping the road, plan drawers, and environmental
personnel coordinate together throughout the planning process to help ensure that the scoping needs get translated onto the
plans and thereby properly accounted for in the permit applications I regret that this has happened We go to great lengths
to try to ensure that we stay in compliance and unfortunately, as in this case, sometimes oversights result in unintentional
impacts As you can see, we have a plan to try to help prevent recurrences of this in the future
