Facility and Address: Mr. Jack H. Campbell Willow Creek Builders, Inc. 510 Hayworth Circle High Point, N.C. 27262 2. Date of Investigation: March 20, 1991 3. Directions to Site: From Winston-Salem take Hwy 109 south to Wallburg High Point Rd. (SR 1741). Turn left and proceed to Abbotts Creek Church Rd. (SR 1743). Turn right and go 1.2 miles and turn right onto Dorado Circle (just before bridge). Go 0.8 mile to crossroad and turn left onto dirt road to plant. 6. Discharge Point - Latitude: 35 58' 04" Longitude: 80 06' 02" Attach a USGS Map Extract and indicated treatment plant site and discharge point on map. USGS Quad No. D18NE or USGS Quad Name High Point West 7. Size (land available for expansion and upgrading): Adequate land is available for expansion and upgrading. 8. Topography (relationship to flood plain included): The treatment plant is located twenty yards from the stream and is in the flood plain. The structure is raised to prevent any damage from flooding. 9. Lpcation of nearest dwelling: There are no dwellings within 1000 ft of the WWTP. 10. Receiving stream or affected surface waters: Abbotts Creek a b c Classification: WS-III River Basin and Subbasin No.: YAD-07 Describe receiving stream features and pertinent downstream uses: The Thomasville and Lexington drinking water supply reservoir, Lake Thom-A-Lex, is located several miles downstream. The Town of Kernersville (NC0021261), Mr. John K. Anderson (SFR NC0070238), Dr. L.S. Averett (SFR NC0047635), Mr. Clinton 0. Bertschi (SFR NC0032255), and Mr. Parks Freeze (SFR NC0047988) discharge upstream of the Willow Creek WWTP. Mr. Kenneth Shull (SFR, NC0057568) RCR Development Lot C (SFR, NC0057576) and Dr. Thomas Lauer (SFR, NC0057550) discharge into Abbotts Creek below the Willow Creek WWTP. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. T pe of wastewater: 100% Domestic % Industrial a. Volume of Wastewater: 0.04 MGD (Existing) Please note that the permit was reissued on 12/19/89 allowing the facility to expand to 0.08 MGD. The A/C was issued on 2/15/91. To date, no construction has occurred. b. Types and quantities of industrial wastewater: N/A c. Prevalent toxic constituents in wastewater: N/A d. Other: PART IV - EVALUATION AND RECOMMENDATIONS The existing 0.04 MGD package WWTP currently receives wastewater from approximately 75 homes and Westchester Academy. The permit was modified on 12/19/89 and the A/C issued o 2/15/91 approving expansion of the facility to 0.08 MGD. The proposed expansion would consist of a new 0.04 MGD package plant to parallel the existing system. wsRp recommends renewal of permit No. NC0034452 to Willow Creek Builders, Inc.
