Inspection Summary:
On April 6, 2021 this facility was visited for an inspection. As in the previous visit, entrance to the facility could not be gained
and all phone numbers posted on exterior main entrance door were not answered when called. This facility will be visited for
an inspection on or after May 25. If a complete inspection cannot be performed, a Notice of Violation will be issued to the
permittee.
The permittee shall assure that all necessary changes are made before the next inspection and modify the program as
necessary to allow for unannounced inspections.
