Dear Mr Matthews
The North Carolina Department of Transportation is scheduled to replace existing Bridge #137
with a reinforced concrete box culvert at the same location The existing bridge is 16 5 feet long
and is located at the intersection of SR 1136 and SR 1137 This is a school bus route and it is
difficult for the buses to make the turn here The culvert option should work well at this location
and will improve the situation for the larger vehicles using this route Stream slope is 4% so we
will have 1' tall buried sills at the inlet, outlet and one in the middle of the culvert to eliminate
head cutting and help retain natural stream alluvium Mitigation will be provided through EEP
No on -site detour is proposed Stream impacts are tabulated below

The replacement of the existing bridge with the proposed new culvert will require temporary
dewatermg An impervious sandbag dike will be constructed approximately 20 feet upstream of
the existing structure to create a small pool Clean water will temporarily be pumped around or
piped through the construction area and will be discharged downstream Any dirty water located
inside the construction area will be pumped into a silt bag if necessary Once the new structure is
in place the impervious dike will be removed and preconstruction streambed elevations will be
restored
Construction of this project will require impacts to waters of the United States in the Watauga
River Basin The existing bridge crosses Clark Creek (Class C) This project was reviewed for
the presence of threatened and endangered species in September 2012 The subject project will
have no effect on any federally listed endangered plant or animal species Historic compliance
work will be scheduled and concurrence with the State Historic Preservation Office will be
obtained if required
The project impacts are not significant in nature and should not alter the best usage for this
stream All appropriate BMPs will be used during construction Attached is a Preconstruction
Notification Form plan sheets a marked county map and a marked topography map
By copy of this electronic letter it is requested that Mrs Marla Chambers with the North
Carolina Wildlife Resources Commission comment directly to you concerning the 404
Nationwide Permit application with a copy sent to me
By copy of this electronic letter it is requested that the Division of Water Quality 401/Wetlands
Unit issue the appropriate permit for construction of this project It is requested that any
comments related to the 401 certification be forwarded directly to me with a copy sent to the US
Army Corps of Engineers
We would like to begin construction as soon as possible If further information is required
