Diffuse Flow and Stormwater ❑ Per the requirements of the Riparian Buffer Rule, you must show that this site meets diffuse flow requirements with a level spreader or other BMP per Chapter 8 of the BMP Manual (see http //portal ncdenr org/web /wq /ws/su/bmp- manual). ❑ Please submit a Level Spreader Supplement Form with all required items for each proposed level spreader (see http / /portal ncdenr org/web /wq /ws /su/bmp- manual) ❑ Please show contour lines down slope of the level spreader to show that it has been correctly sited per the BMP Manual. ❑ The slope from the proposed level spreader(s) lip ( ) to the top of bank of the stream exceeds the allowable slope for type of vegetated filter strip (5% for riparian buffer VFS, and 8% for engineered filter strip). You must either relocate the level spreader(s) to a location where the filter strip slope(s) is /are appropriate or provide a BMP that removes 30 percent TN and TP. For each proposed BMP, provide a completed BMP Supplement Form with all the required items (http / /portal ncdenr org/web/wq/ws/su/bmp- manual) ❑ Please provide calculations to show the outlet velocity from outlet(s) to the wetland(s). A non - erosive velocity of two feet per second or less must be achieved for the peak flow from the 10 -year storm. ❑ [Drainage Area Map and Percent Imperviousness Breakdown] ❑ Please delineate the drainage areas on the site plan (Drainage area map on full plan sheets). ❑ Please provide the overall percent imperviousness of the project, and an inventory of impervious surfaces in each drainage area. The inventory should include all proposed building footprints, roads, driveways, sidewalks, gravel - surfaced areas, amenity areas, etc. ❑ [High Density] Per the requirements of GC , this project is subject to Stormwater Management Plan (SMP) Requirements for Applicants Other Than the North Carolina Department of Transportation (see http //portal ncdenr org/ web /wq /swp /ws/ 401 /certsandpermrts /swmanagement). For each proposed BMP, please provide a completed BMP Supplement Form with all required items (see http / /portal ncdenr org/web /wq /ws /su/bmp- manual). ❑ [Low Density] Per the requirements of GC , this project is categorized as low density. For each vegetated stormwater conveyance, please provide a completed Low Density Supplement Form along with all supporting information (see http / /portal ncdenr org/web /wq /ws /su/bmp - manual). ❑ Please provide a BMP Supplement Form with all required items for every BMP on the site (see http / /portal ncdenr org/web/wq/ws/swbmp- manual). ❑ Please use the most current version of the BMP Supplement Form (see http / /portal ncdenr org/web /wq /ws / su/bmp- manual). Mitigation ❑ of compensatory mitigation is required for this project. Please provide a compensatory mitigation plan. The plan must conform to the requirements of 15A NCAC 2H .0500 and must be appropriate to the type of impacts proposed. ❑ Buffer mitigation is required for the proposed buffer impacts. Please submit a mitigation plan detailing the amount of buffer impact for Zone I and/or Zone 2 and how the impact will be mitigated. ❑ Please provide documentation from an appropriate mitigation bank or the NCEEP indicating their willingness to accept your request to purchase (type of mitigation) credit to mitigate for the proposed (type of mitigation) impacts listed in your PCN application per Section D. Mitigation provider selection must conform to the requirements of G.S. 143 - 214.11 (for wetland and stream mitigation) or G.S. 143- 214.20 (for riparian buffer mitigation). Other ❑ The application fee is insufficient because over 150 feet of stream and/or over 1 acre of wetland impacts were requested. Please provide $ . This additional fee must be received before your application can be reviewed. ❑ Please provide a signed copy of the application. ❑ Please provide an agent authorization letter for the application signature. ❑ Please indicate which 404 Permit the USACE has authorized for this project. ❑ Please resubmit copies of your information on the January 2009 PCN form or a more recent version. Please resubmit the application page of the variance application with the owner's signature. ❑ The proposed buffer impacts in your application require a major variance from the Environmental Management Commission. Please submit 3 copies of your information of the July 2009 Variance Application Form with supporting documentation. ❑ Please provide one (1) data CD of full size plans in TIFF Group 4 format (black and white, not grayscale or color). Stormwater Management Plan (SMP) for Future Phase(s) You are required to provide one copy of the approved SMP for the future phase(s), including plan details on full -sized plan sheets, with proof of Durham County's approval. (Please note that even "low density" development constitutes an SMP.) The approved SMP must be submitted to the DWQ Webscape Unit (1650 Mail Service Center, Raleigh, NC 27699 -1650) before any land disturbance associated with that phase may occur. The following requirements also apply: • The approved SMP for the future phase(s) must be constructed and operational before any permanent building or other structure is occupied at the site. • The approved SMP for the future phases(s) as well as drainage patterns must be maintained in perpetuity. • The SMP may not be modified without prior written authorization from Durham County. A copy of the approval letter and the modified SMP must be submitted to the DWQ Webscape Unit prior to the commencement of the modifications.
