INSTRUCTIONS FOR FORM: FTA 04-16 & SUPPORTING DOCUMENTATION
This application is for sewer extensions involving gravity sewers, pump stations and force mains, or any combination that has been
certified by a professional engineer and the applicant that the project meets the requirements of I SA NCAC 02T and the Division's
Minimum Design Criteria and that -plans, specifications and supporting documents have been prepared in accordance with,, 15A
NCA , 15A NCAC 02T.0300. Division Policies and good engineering practices.
While no upfront engineering design documents are required for submittal, in accordance with 15A NCAC 02T .0305(b), design
documents must be prepared prior to submittal of a fast track permit application to the Division. This would include plans, design
calculations, and project specifications referenced in 15A NCAC 02T .0305 and the applicable minimum design criteria. These
documents shall be available upon request by the Division.
Projects that are deemed permitted (do not require a permit from the Division) are explained in 15A NCAC 02T.0303.
Projects not eligible for review via the fast track process (must be submitted for full technical review):
➢ Projects that require an environmental assessment in accordance with 15A NCAC 1 C .010
➢ Projects that do not meet any part of the minimum design criteria (MDC) document; C Dept of Environmental Quality
➢ Projects that involve a variance from the requirements of I5A NCAC 2T;
➢ Pressure sewer systems utilizing septic tank -effluent pumps (STEPS) or simplex grinder pumps; MAR - 5 2021
➢ STEP or simplex grinder pumps connecting to pressurized systems (e.g. force mains);
➢ Vacuum sewer systems.

Dear Sir or Madam:
Johnston County intends to make improvements to its sanitary sewage pumping station and equalization
facility located at the Johnston County Landfill at 68o County Home Road, Smithfield, NC 27577. The
existing facility consists of a submersible sewage pumping station, mechanical screen, Parshall Flume,
equalization lagoon and a 12-inch force main to the Central Johnston County Wastewater Treatment Plant
(WWTP). Johnston County intends to construct the following general improvements including:
• Addition of a flow distribution box
• Addition of a new, larger submersible station with valve vault
• Addition of a 24-inch force to the future HWY 210 WWTP
• Relocation of the existing generator
• Addition of a new electrical building
• Addition of a 12 mgd grit removal
• Reconstruction of the existing lagoon
Therefore, this application is for modifying the existing pumping station under permit #WQ00203040.
The existing submersible pumping station will be demolished, and a new larger submersible station
constructed. The concrete pumping station structure will be sized to accommodate five pumps, but only
four out of five submersible pumps will be initially installed to achieve the 8.75 mgd interim capacity. At a
future date, when flow is anticipated to exceed 8.75 mgd, the 5`h pump will be installed and a second
mechanical screen and larger generator will be added such that the station's firm capacity meets the
projected build -out capacity of n.96 mgd. Addition of a 5th pump, second screen and generator will
require another permit modification in the future and is not included in this current permit modification.
The County plans construction of the HWY 210 WWTP starting in 2021. Until the HWY 210 WWTP is
operational, the proposed pumping station will continue to pump through the existing 12-inch force main
at 1,340 gpm to the existing Central Johnston County WWTP. Once the HWY 210 WWTP is operational,
the proposed pump station will initially convey up to 8.75 mgd to the new WWTP. Please note that the
existing 12-inch force main to the Central Johnston County WWTP will remain in service and be used on
an as needed or emergency basis.
Furthermore, the build out capacity of the pumping station through the proposed 24-inch force main is
estimated to be 11.96 mgd (8,309 gpm). To convey this flow, a 5u' pump would be installed in the future
to achieve the firm capacity of 11.96 mgd. At that point, 4 of 5 pumps operating simultaneously and in
parallel achieve the 11.96 firm capacity, or in other words, each pump's capacity would be approximately
2,07.7 gpm at 181 feet total dynamic head (TDH). Thus, the currently proposed pumps would not require
replacement, but rather with addition of a 5u' pump and larger generator the higher firm station capacity
can be achieved.

This pumping station receives flow from multiple other pumping stations and has a permitted
equalization lagoon. Thus, with the addition of the flow distribution box, the influent wastewater can be
diverted to either the equalization lagoon (primary mode of operation) or proposed pumping station or
divided to both at the same time. When wastewater is diverted to the equalization lagoon, it will first go
through a previously permitted mechanical screen (8.75 mgd capacity) and then through the proposed 12
mgd rated grit removal unit before then flowing into the equalization lagoon. This approach will reduce
solids loading in the lagoon. Then, from the equalization lagoon, the wastewater will then drain to the
proposed pumping station. Improvements to clean the equalization lagoon include a new entrance ramp,
concrete slab on the bottom, sump pump, and water cannons.

In accordance with your application received October 25, 2007, we are forwarding
herewith modified Permit No. WQ0020340 dated September 19, 2008, to Johnston County for
the construction and operation of the subject wastewater transmission facilities.
This modified permit shall void Permit No. WQ0020340 previously issued on June 13,
2003, shall be effective from the date of issuance until rescinded, and shall be subject to the
conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are unacceptable, you
have the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request must be in the form of a written petition, conforming to
Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative
Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714. Unless such demands are made
this permit shall be final and binding.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility
has been installed in accordance with the approved plans and specifications. Mail the
Certification to the Construction Grants & Loans Section, 1633 Mail Service Center, Raleigh,
North Carolina 27699-1633. (Engineer's Certification Form is attached.)

Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction and operation of the Landfill Equalization Facility consisting of a mechanical bar
screen rated at 8.75 mgd with a manual bypass bar screen; a 12-inch Parshall Flume;
approximately 450 linear feet of 24--inch gravity sanitary sewer line; utilization of the existing
lined solid waste landfill basin as a wastewater equalization basin with an approximate total
volume of 5.1 million gallons (basin bottom to maximum water level with 2-feet freeboard) of
which approximately 900,000 gallons (basin effluent to high water level) will be utilized for flow
equalization; approximately 180 feet of 8-inch gravity landfill leachate sewer line; with no
increase in volume of flow into the Johnston's County's existing sewerage system, pursuant to
the application received October 25, 2007, and in conformity with the project plan,
specifications, and other supporting data subsequently filed and approved by the Department of
Environment and Natural Resources and considered a part of this permit.
This permit also grants permission for the:
continued operation of approximately 27,760 linear feet of 12-inch force main for raw sewage
and 26,930 linear feet of 8-inch force main for waste activated sludge installed in a common
trench between the Central Johnston County Regional Wastewater Treatment Facility
(NC0030716) and the Johnston County Landfill and continued operation of a 1,340 GPM sewage
pump station with duplex submersible pumps, high-water alarm with remote telemetry, on -site
standby power, and approximately 5,400 linear feet of 12-inch force main for transmission of
wastewater from McGees Crossroads and Cleveland areas via connecting force mains directly to
the Central Johnston County Regional Wastewater Treatment Facility (NC0030716) with no

increase in volume of flow to the existing sewerage system, pursuant to the permit issued on
June 13, 2003, and in conformity with the project plan, specifications, and other supporting data
subsequently filed and approved by the Department of Environment and Natural Resources and
considered a part of this permit.
This Modified Permit shall void Permit No. WQ0020340, issued on June 13, 2003, shall
be effective from the date of issuance until rescinded, and shall be subject to the following
conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in accordance with
the conditions of this permit, the approved plans and specifications, and other supporting
data.
2. This permit is effective only with respect to the nature and volume of wastes described in
the application and other supporting data.
3. The wastewater collection facilities shall be properly maintained and operated at all
times. The Permittee shall maintain compliance with an individual system -wide
collection system permit for the operation and maintenance of these facilities as required
by 15A NCAC 2T .0400. If an individual permit is not required, the following
performance criteria shall be met as provided in 15A NCAC 2T .0400:
(a) The sewer system shall be effectively maintained and operated at all times to prevent
discharge to land or surface waters, and to prevent any contravention of groundwater
standards or surface water standards.
(b) A map of the sewer system shall be developed and actively maintained.
(c) An operation and maintenance plan including pump station inspection frequency,
preventative maintenance schedule, spare parts inventory and overflow response
shall be developed and implemented.
(d) Pump stations that are not connected to a telemetry system (i.e., remote alarm
system) shall be inspected by the permittee or its representative every day (i.e.,
365 days per year). Pump stations that are connected to a telemetry system shall
be inspected at least once per week.
(e) High -priority sewers shall be inspected by the permittee or its representative at
least once every six -months and inspections are documented.
(f) A general observation by the permittee or its representative of the entire sewer
system shall be conducted at least once per year.
(g) Overflows and bypasses shall be reported to the appropriate Division regional
office in accordance with 15A NCAC 02B .0506(a), and public notice shall be
provided as required by G.S. I43-215.1C.

(h) A Grease Control Program shall be established as follows:
(i) For publicly owned collection systems, the Grease Control Program shall
include at least bi-annual distribution of educational materials for both
commercial and residential users and the legal means to require grease
interceptors for new construction and retrofit, if necessary, of grease
interceptors at existing establishments. The plan shall also include legal
means for inspections of the grease interceptors, enforcement for violators
and the legal means to control grease entering the system from other
public and private satellite sewer systems.
(ii) For privately owned collection systems, the Grease Control Program shall
include at least bi-annual distribution of grease education materials to
users of the collection system by the permittee or its representative.
(iii) Grease education materials shall be distributed more often than required in
Parts (i) and (H) of this Subparagraph if necessary to prevent grease -
related sanitary sewer overflows.
(i) Right-of-ways and easements shall be maintained in the full easement width for
personnel and equipment accessibility.
(j) Documentation shall be kept for Subparagraphs (a) through (i) for a minimum of
three years with exception of the map, which shall be maintained for the life of the
system.
4. The sewage and wastewater collected by this system shall be treated in the Central Johnston
County Regional Wastewater Treatment Facility (NC0030716) existing sewerage system
prior to being discharged into the receiving stream.
5. This permit is not transferable. In the event there is a desire for the facilities to change
ownership, or there is a name change of the Permittee, a formal permit request must be
submitted to the Division of Water Quality (Division) accompanied by an application fee,
documentation from the parties involved, and other supporting materials as may be
appropriate. The approval of this request will be considered on its merits and may or may
not be approved.
6. Construction of the sewers, pump station(s) and force main shall be scheduled so as not to
interrupt service by the existing utilities nor result in an overflow or bypass discharge of
wastewater to the surface waters of the State.
7. A copy of the approved plans and specifications shall be maintained on file by the Permittee
for the life of the wastewater collection facilities.

8. Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted
facility has been installed in accordance with this permit, the approved plans and
specifications, and other supporting materials. If this project is to be completed in phases
and partially certified, you shall retain the responsibility to track further construction
approved under the same permit, and shall provide a final certificate of completion once the
entire project has been completed. Mail the Certification to the Construction Grants &
Loans Section, 1633 Mail Service Center, Raleigh, North Carolina 27699-1633.

9. Failure to abide by the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division, in accordance with North Carolina
General Statute 143-215.6A to 143-215.6C.
10. The issuance of this permit does not exempt the Permittee from complying with any and
all statutes, rules, regulations, or ordinances that may be imposed by other government
agencies (i.e., local, state, and federal) having jurisdiction, including but not limited to
applicable river buffer rules in 15A NCAC 2B .0200, soil erosion and sedimentation
control requirements in 15A NCAC Chapter 4 and under the Division's General Permit
NCG010000, and any requirements pertaining to wetlands under 15A NCAC 2B .0200
and ISA NCAC 2H .0500.
11. The Permittee shall provide for the pump station and force main the following items:
(a) Pump Station Reliability:
(i) Pump stations, except when exempted by Subparagraph 0)(2) of 15A
NCAC 02T.0305, shall be designed with multiple pumps such that peak
flow can be pumped with the largest pump out of service.
(ii) A standby power source or pump is required at all pump stations except
for those pump stations subject to Subparagraph 0)(2) of 15A NCAC
02T.0305. Controls shall be provided to automatically activate the
standby source and signal an alarm condition.
(iii) As an alternative to Part (ii) for pump stations with an average daily
design flow less than 15,000 gallons per day as calculated using 15A
NCAC 02T.0114, a portable power source or pumping capability may be
utilized. It shall be demonstrated to the Division that the portable source
is owned or contracted by the permittee and is compatible with the
station. If the portable power source or pump is dedicated to multiple
pump stations, an evaluation of all the pump stations' storage capacities
and the rotation schedule of the portable power source or pump, including
travel timeframes, shall be provided in the case of a multiple station power
outage.

(iv) As an alternative to Part (ii) for pump or vacuum stations connecting a
single building to an alternative sewer system, wet well storage
requirements shall be documented to provide 24-hours worth of
wastewater storage or, exceed the greatest power outage over the last three
years or the documented response time to replace a failed pump,
whichever is greater. Documentation shall be required pursuant to the
permit application.
(v) All pump stations designed for two pumps or more shall have a telemetry
system to provide remote notification of a problem condition to include
power failure and high water alarm.
(vi) High water audio and visual alarm.
(b) Pump stations shall have a permanent weatherproof sign stating the pump station
identifier, 24-hour emergency number and instructions to call in case of
emergency. Simplex pump or vacuum stations serving a single-family residence
shall have a placard or sticker placed inside the control panel with a 24-hour
emergency contact number.
(c) Screened vents for all wet wells.
(d) The public shall be restricted access to the site and equipment.
(e) Air relief valves shall be provided at all high points along force mains where the
vertical distance exceeds ten feet.
12. In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those
that may be required by this Division, such as the construction of additional or
replacement wastewater collection facilities.
13. The facilities shall be constructed in accordance with the requirements of the North
Carolina Sedimentation Pollution Control Act.
14. If applicable, the Operational Agreement between the Permittee and the
Environmental Management Commission is incorporated herein by reference and is
made a condition of this Permit. Noncompliance with the terms of the Operational
Agreement may subject the Permittee to an enforcement action by the Division in
accordance with North Carolina General Statute 143-215.6A to 143-215.6C. A copy
of the Articles of Incorporation, Declarations, and Bylaws that have been properly
filed shall be submitted with the engineer's certification prior to operation.
15. Any abandoning of the existing facilities must be done in accordance with the
requirements of the Division of Water Quality's Mooresville Regional Office.

16. Noncompliance Notification:
The Permittee shall verbally report to a water quality staff member at the Mooresville
Regional Office, telephone number [phone], as soon as possible, but in no case
more than 24 hours or on the next working day following the occurrence or first
knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that render the
facility incapable of adequate wastewater transport such as mechanical or
electrical failures of pumps, line blockage or breakage, etc.
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to
receiving waters without treatment of all or any portion of the influent to such
station or facility.
Voice mail messages or faxed information is permissible but this shall not be considered
as the initial verbal report. Overflows and spills occurring outside normal business hours
may also be reported to the Division of Emergency Management at telephone number
[phone] or [phone]. Persons reporting any of the above occurrences shall
file a spill report by completing Part i of Form CS-SSO (or the most current Division
approved form), within five days following first knowledge of the occurrence. This
report shall outline the actions taken or proposed to ensure that the problem does not
recur. Per Condition I(2), Part II of Form CS-SSO (or the most current Division
approved form) can also be completed to show that the SSO was beyond control.

A couple more questions I should have asked earlier:
1. Item IX.2 is marked "no" for compliance with %i,ctland separation requirements. Please
explain. There is a small portion (less than 0.1 acres) of the proposed force main that will go through
a wetlands area. The required DWR 401 and USACE 404 permit have been submitted. The USAGE
404 permit has been approved and the DWR 401 is still under review.
2. Item IX.G is marked "no" for high -priority lines. We just want to confirm this, since there will over a
mile of new line. Correct. No portion of the proposed force main tits the definition per 15A NCAC
02T.0402.

Hi Steve,
Ok, it sounds like this will go through the fast -tack process.
This application is a little odd, staff in my office has never done one like this before, to my
knowledge. We've never modified an existing permit that was issued by another department (construction
grants and loans) that includes more items than just wastewater gravity and forcemain lines and typical pump
stations.
As with all pen -nit modifications, the modified permit we issue will completely replace, !rescind the older
pen -nit, so the new modified permit will need to contain everything that was in the original permit that will
remain in operation. So here's what was in the original pen -nit:
• Mechanical Bar Screen rated at 8.75 mgd with a manual bypass bar screen This will remain
. 12"Parhsali Flume This will remain
• 450 LF of24" gravity sewer line This will remain
• Utilization of an existing lined solid waste landfill basin as a wastewater equalization basin with an
approximate total volume of 5.1 million gallons (basin bottom to maximum water level with 2-feet
freeboard) of which approximately 900,000 gallons (basin effluent to high water level) will be utilized
for flow equalization 'Chis will remain and there will be a couple ne« pipe penetrations and a concrete
topper placed in the bottom to smooth out the bottom for better cleaning.
• Approximately 180 LF of 8" gravity landfill leachate sewer line This will remain as leachate will be
redirected to the distribution box.
• 27,760 LF of 12" FM for raw sewage this will remain
• 26,930 LF of 8" FM for Waste Activated Sludge installed in a common trench This will remain
• Continued operation of 1,340 GPM sewage pump station with duplex pumps This will be demolished in
favor of the proposed 6,076 gpm station.
• 5,400 LF of 12" FM for transmission of wastewater from McGee's Crossroad and Cleveland area via
connecting force mains directly to CJCRWWTF. This will remain, as it connects to the 27,760 If of
12" that conveys sewage to the ex WWTP.
So again, if all this is staying in operation. we'll need to include all this in the new permit %ve issue (because it
completely replaces the old permit). Please let me know if any of the above is being removed or modified.
(We'll probably need to add the grit removal and flow splitter box). Ok. we'll add the grit and FSB and
interconnecting 30" pipe as well.
Also, the new application included 7,000 LF of 24" FNI. Is this all new sewerline? yes, as it will convey
wastewater to the proposed Hwy210 WWTP once in operation.

The new application also includes a 6,076 GPM PS. IS this replacing the existing 1,340 GPM
PS? Yes And it will be a fourplex PS? Yes, we are installing 4 pumps in the 5 slot pumping station to
achieve 6,076 gpm (8.75 mgd) capacity (3 duty/l spare).

As we discussed with Christyn today,
The lagoon volume is not being increased. The ex lagoon is made of FabriForm concrete (looks like a quilt
blanket) and not flat. To help facilitate solids removal, we are placing a concrete `topper' in the bottom to
create a flat surface. Once dewatered, the County can scrape the bottom of the lagoon with a front-end loader
to remove the solids far easier with a flat surface than the current rippled concrete surface. There are also a
couple pipe penetrations we are adding to the lagoon. One is to fill it from the proposed grit unit. The other
is a sump to better drain the liquid from the bottom. And a better truck ramp to drive in/out of it.
Regarding the flow splitter box, wastewater currently enters the site through force mains. All three force
mains will be redirected to a `splitter box'. This box has two weirs in it. Depending which one is up!down,
influent wastewater will go directly to the proposed wet well or to the screen/grit removal unit/lagoon and
then to the wet well via a flow control valve.

I had initially talked with Christyn before I sent my first email, and it was through those discussions that she
determined this particular project needed to go through a full technical review.
I've since followed -up with her and reminded her that you had discussed this with her back in January. She
recalled the conversation, and went through her notes. She recalls the conversation being about whether this
project could be considered a sewer extension, or if it would require an Authorization to Construct. She did
not recall a discussion about redoing/altering some of the components, specifically the now diversion box and
the EQ basin. Because of those changes, it is not eligible for a fast -track and needs to go to her for a full
re%- iew.

is appropriate for this project. Attached is her confirmation that a FastTrack permit is appropriate. Based
upon that meeting we moved forward with a FastTrack submittal and established a schedule to advertise for

We received the subject application 3-5-21. The app notes this is a mod to permit WQ0020340. However,
our records note this permit was previously issued with the project name "McGees Crossroads".
Can you confirm the name for this project, and the correct permit number?
Once the correct name is confirmed, we may have some other questions regarding the app. As you know, for
mod apps, the original permit becomes void, and the modified permit must contain everything in the original
permit that will remain in operation, plus any additional sewer lines, pump stations, or flow.
