This compliance inspection was conducted by DWR staff person Patrick Mitchell. The inspection reflects compliance with Permit No. WQ0038087. This inspection only consisted of a review of land application site activities and a review of the Mathis storage site under construction; none of the other source facilities were visited during this inspection. Accompanying staff on the inspection was Mr. Luke Mathis, ORC and Mr. David Coyle, LSS of Terra Renewal. The inspection reflects compliance with the permit. Below is a summary of notes from this inspection. Land Application Site Review: • Land application site WK-CH was reviewed during this inspection. Only field 03 had been utilized with buffers flagged at the time of inspection. • No land application was occurring at the time of inspection. Reportedly the residuals that were present in the frac tank present in the staging area had been land applied prior to the inspection and was empty. The operator indicated that he was waiting on tanker trucks to deliver additional residuals loads to the site for spreading. No transport was observed during the inspection. **Plan to conduct a follow up inspection to review activities in operation.** • Observed land application activities were appeared to be compliant (buffers met, incorporated, depth not exceeding the maximum of 6" for field 03, access restriction sign present, etc.). • Residuals from Tyson -Wilkesboro and Tyson -Harmony were the two sources reportedly being applied during this event. Representatives indicated that residuals analyses have been completed (except TCLP — to be done later this year) and Fecal Coliform results meet Class B Pathogen Reduction requirements, but none of these results were not available for review during inspection. Rquest that these results be emailed. • Reported PAN loading and metals loading appear to be compliant Onsite records show that carryover PAN is being considered for determining event loading rates. Again, residuals analyses lab results were not present during the inspection to confirm. Request that these results be emailed. • Soils sampling has reportedly been completed for site WK-CH, results were not available for review during the inspection. Request that these results be emailed. Mathis Storage Site Review: • The secondary containment berm has been installed. The stormwater outlet feature is still under construction. • The footing was being dug at the time of inspection, in preparation for pouring the concrete foundation for the storage structures. • Reminded representatives to notify WSRO when construction is completed to allow inspection before being put into use.
