Mr. Jimmy Boles and Mr. Chad Whitherspoon both with LP Corp. accompanied staff on the inspection. The activiies reviewed during the inspection reflect compliance with the permit. The only deficiency observed is the continued nitrate levels >2L limits in the groundwater monitoring of former dedicated land application fields. Below is a summary of notes from this inspection. • The facility continues to produce no residuals and no land application activities under this permit Reportedly the last residuals activities conducted under this permit was in May 2020. • The facility continues to conduct groundwater monitoring of former dedicated land application fields as required in the Attachment C of the permit. • Data reviewed during the inspection suggests that MW-1, MW-5 and MW-9 are the only wells that continue to indicate elevated nitrate levels. MW-4 and MW-6 are consistently below the 2L limit for nitrate. See field map sketch for locations and notes from inspection. • The facility is still in the process of conducting the investigation of elevated nitrate levels in groundwater at the former dedicated land application fields as required in the schedules section of the permit. Reminded representatives that the investigation report and a corrective action plan are due on or before May 17, 2021. Follow up inspection is needed for site review once the investigation and corrective action plan are reviewed.
