Dear Mr. Cranston:
Staff of the North Carolina Division of Water Resources Winston-Salem Regional Office (DWR)
has completed a review of the 2020 Annual Report for the subject facility. This review was
completed by DWR staff person Patrick Mitchell. The majority of the review reflects compliance
with Permit No. WQ0000461. However, the following deficiency warrants your continued
attention and action:
The groundwater sampling results for three of the monitoring wells (i.e., MW-1, MW-5, and
MW-9) serving the former dedicated land application fields continue to show Nitrate
concentrations greater than the NC Groundwater Standard limit of 10.0 mg/L. Please be
reminded that Permit Condition I.1. requires that an investigation shall be conducted
and a report containing a corrective action plan shall be submitted to the Division within
180 days of permit issuance (i.e., on or before May 17, 2021).

Reason for Visit: Routine
Inspection Summary:
On March 30, 2021, WSRO staff conducted a review of the subject facility's 2020 Annual Report. No site visit was made as
part of this inspection event. The majority of the report reflects compliance with the subject permit. However, groundwater
sampling results for the former dedicated fields shows exceedances for Nitrate in three of the monitoring wells.
Below is a summary of notes from this review.
• Monitoring wells MW-1, MW-5, and MW-9 all exceeded the groundwater limit of 10 mg/L for Nitrate.
• Copper and Zinc were reported as elevated (i.e., >2,000 index value) for site sample "WHRLP". It should be noted that no
land application was reported as occurring after March 31, 2020 and the new permit is for inactive (i.e., 0 residuals
production or distribution). Therefore, no land application should be occurring from the subject facility. If this site is returned
to use, soil sampling should be completed to ensure Copper and Zinc levels are not too high.
• Residuals analyses results provided in the 2020 report met regulatory limits.
• Field loading records suggests that all land application that occurred in 2020 was conducted within agronomic loading
limits.
