Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This final permit includes no major changes from the draft permit sent to you on July 21, 2004.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days
following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter
150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail
Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final
and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local
governmental permit that may be required. If you have any questions concerning this permit, please contact

This permit and authorization to discharge shall expire at midnight on August 31,2009.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or
discharge are hereby revoked. As of this permit issuance, any previously issued
permit bearing this number is no longer effective. Therefore, the exclusive authority
to operate and discharge from this facility arises under the permit conditions,
requirements, terms, and provisions included herein.
Columbus County Schools is hereby authorized to:
1. Continue to operate an existing 0.005 MGD wastewater treatment facility with
the following components:
• 2, 000-gallon grease trap
• 8, 000-gallon septic tank
• Dual 3,000-gallon recirculation tanks
• Dual recirculation / discharge pumps
• 1250-ft2 recirculating sand filter
• Ultraviolet disinfection system
• Effluent Flow Meter
This facility is located at Old Dock Elementary School, South of Old Dock on New
Britton Highway East in Columbus County.
2. Discharge from said treatment works at the location specified on the attached
map into an unnamed tributary to Gum Swamp Run, classified C-Swamp waters
in the Lumber River Basin.

includes a 2000 gallon grease trap, 8000 gallon septic

filter, filter recirculation pumping station containing
dual pumps and dual 3000 gallon recirculation tanks,UV
disinfection system, effluent flow meter, high water
alarms and recirculation system logic circuitry, and
gravity discharge piping. The first filter dose is via
(above ground) low pressure pipe and recirculation doses
are via a perc-rite w-31-s low profile spray system.

PART IV - EVALUATION AND RECOMMENDATIONS
This report is being prepared based upon a permit renewal
request. The wastewater treatment system was upgraded in the
late 90's via requirements of an SOC. The discharge is to a
zero flow stream. The stream was dry at the downstream
monitoring point on the inspection date. The effluent was very
clear and odorless.
The current effluent limits for the existing facility
appear to meet the requirements of NCAC 2B.0200, Section .0206
for zero flow streams. All samples are taken by grab method.
Monitoring frequency is not exactly per NCAC 2B.500 concerning
water quality limited cases for SIC 8211. However, the writer
does not recommend change because the effluent limits may have
been agreed upon during the SOC. A records review shows the
plant to be in substantial compliance with the permitted
effluent limits (including NH3-N).
The permit contains old language within Condition
B.1.c. relative to state enforcement which should be changed
from $10,000 to $25,000 for civil penalties.
It is recommended that NPDES Permit No. NC0043745 be
prepared and published as a notice to the public. If no
significant adverse public comment is received, it is
recommended that NPDES Permit No. NC0043745 be reissued for a
standard term per the Lumber River basinwide strategy to the
Columbus County Schools.
