The performance evaluation (PE) results submitted by your laboratory from the NSI WP-107 Performance
Evaluation Study are summarized below.. A "Fail" entry, requires a follow up sample to be analyzed and reported
to this office within 60 days. A date entered beside the "Fail" entry represents the completion deadline for this PE
follow up.

Since no follow up results were submitted this is considered a second successive failure. In accordance with-15A
NCAC 2H .0807 (a) (10) and (b) (1) your laboratory may be decertified for both parameters. Your laboratory
will be allowed 30 days to produce a passing result for .both parameters. Contact your -PE provider and
request a rapid response type of sample. This must be a blind sample just as the WP.,Failure to analyze and
submit an acceptable follow up sample result in the allotted time or producing another unacceptable result will
result in decertification for that parameter.
You are reminded that an acceptable performance evaluation (PE) result must be obtained 'for all certified
parameters which are available from the accredited vendors each year. This letter pertains to your •2005 PE
sample results only.
