performed an interim inspection for the federally funded portion of the stibject project on
September 19, 2006. This inspection was conducted in conjunction with the monthly progress meeting.

E. The Following Work Was Observed to Be Completed
1. Contract 1 Cross Creek WWTP
The pavement base coarsehas been placed for aproximately 70% of the road
surfaces. The removal of the subsurface material has been completed for the
remainder of the road surfaces. The base coarse has been completed for the
remainder of the road surfaces. The curb and guttet replacement has begun. The
curb and gutter replacement has been completed. The overlay has been
completed. Additional overlay change order #2 ha.s also been completed.
2. Contract 2 Rockfish Creek WWTP
a. Junction Box
Concrete has been placed for the base slab and the bottom half of the
walls. The •reinforcing steel and approximately 50% of the formwork
have been erected for the top half of the walls: Backfill has been
placed for approximately 50% of the structure; The remainder of the
formwork has been completed and concrete has been placed for the top
half of the walls. The reinforcing steel and fotinwork for the top slab
have been erected. Concrete has been placed for the top slab, all the
formwork has been removed and backfill has been placed around the
structure. Concrete has been placed for a removable concrete slab on
top of the structure. Approximately 90% of tfie waterproof coating has
been applied to the interior walls. The remainder of the waterproof
coating has been applied to the interior walls. The influent bypass
pumping has been completed and the cutout Of the 54-inch influent
pipe inside the junction box has been completed.
b. New Influent Pump Station
The concrete has been placed for the sloped base slab for the screw
pumps. Concrete has been placed for the bottotn half of the screw

pump exterior wall at elevation 76.50 feet. The reinforcing steel and
approximately 50% of the formwork have been erected for the top half
'of the screw pump exterior walls at elevation $7.00 feet. The
remainder of the formwork has been erected and concrete has been
placed for the top half of the screw pump exterior walls at elevation
87.00 feet. Concrete has been placed for the Sdtith exterior wall.
Reinforcing steel and 50% of the formwork have been erected for the
South interior wall. Approximately 25% of the formwork has been
erected for the screw pump channel walls. The remainder of the
reinforcing steel and formwork has been completed and removed from
the South interior wall. Concrete has been played for the suspended
slab at elevation 82.00 feet at the South end. Concrete has been placed
for the sloped base slab up to elevation 79.50 feet. Concrete has been
placed for three of the four interior screw puriji channel walls up to
elevation 99.83 feet. Approximately 50% of the forms has been
erected.for the 4`i' interior screw pump wall arid the West exterior wall.
Concrete has been placed for the North wall. Concrete has been
placed for the remaining walls and the forms have been removed. The
excavation has been completed and subgrade stone has been placed for
the stairs on the West side of the structure. CCficrete has been placed
for the lower stairs and walls and backfill has been placed. Concrete
.and backfill have been placed for the upper stairs and walls. Concrete
'has been placed for the motor/gearbox pads. Approximately 90% of -
the waterproof coating has been applied to the interior walls.. The
remainder of the waterproof coating has been applied to the interior
walls. Grout fill has been placed in the structure. The screw pumps
have been set in place. Approximately 70% Of the ductwork, stairs,
grating and handrails have been completed. 'I' he remainder of the
aluminum stairs and grating has been completed. Approximately 95%
of the ductwork for the wet well has been corripleted. The remainder.
of the ductwork for the wet well has been cortipleted. Grouting has
been completed on screw pumps #1 and #2 and they have been placed
in service. Screw pumps #3 and #4 have beeii placed into service.
c. Screening Facility
The exterior wall forms have been erected for the bottom section of
walls. Concrete has been placed for the bottom section of walls and
reinforcing steel has been erected for the rem t hing sections of exterior
walls. Concrete has been placed for the base slab and the
interior/exterior walls to elevation 92.17 feet aliid the forms have not
been removed. The remainder of the wall foriiis has been removed
and approximately 50% of the forms and reinforcing steel has been
erected for the suspended slab at elevation 93:t)0 feet. The remaining
,forms and reinforcing steel have been erected for the suspended slab
and concrete has been placed for the suspended slab. -Grout fill has
been placed in the structure. The slide gates afid_flumes have been set
in place. The. flumes have been grouted.. The interior walls have been
prepped for Waterproofing. The waterproofing has been completed

' and the form and bollards have been set in place for the dumpster pads.
The remainder of the work on the slide gates lids been completed.
Concrete has been placed for the dumpster pads. The yard hydrants
have been completed. Startup has been completed and the equipment
has been placed into service.
d. Vortex Grit Removal Facility
The demolition of aeration basin #1 drain pipe has been completed.
The excavation has been completed.' Concrete has been placed for the
base slab at elevation 68.00 feet. The interior Wall forms have been
erected and approximately 40% of the reinforcing steel has been
erected for the West wall (to elevation 92.33 feet). Approximately
50% of the forms have been erected for the East wall to elevation
75.00 feet. Concrete has been placed for the East wall to elevation
75.00 feet. Concrete has been placed for the SCiuth, West and North
walls to elevation 92.33 feet. The fine grade and stone has been
completed at elevation 70.33 feet. Approximately 80% of the backfill
has been placed for the West.wall(to elev. 78:83 feet) and East wall
(to elev. 74.33 feet). The remainder of the bacl{fill has been placed for
the West wall. Concrete has been placed for the influent base slab at
elevation 81.00 feet and the sloped suspended slab between elevations
81.00 feet and 76.50 feet. Concrete has been _ Slaced for the influent
channel walls and the grit vortex collector watts to elevation 81.17
feet. Backfill has been placed and fine grading has been placed to
elevation 81.17 feet. The forms have been refiioved from the effluent
channel walls. Concrete has been placed for the remaining exterior
walls to elevation 92.17 feet. Approximately 5t% of the forms and
reinforcing steel has been erected for the suspended slab at elevation
93.00 feet. The remaining forms and reinforeing steel has been
erected for the suspended slab and concrete has been placed for the
suspended slab. The slide gates have been set in place. The interior
walls have been prepped for waterproofing. Approximately 80% of
the waterproofmg has been completed. The vdttex grit collectors have
been installed. The remainder of the grit collection equipment has
been installed and the waterproofing has been completed. The HVAC,
lights and electrical boxes have been completer) and concrete has been
placed for the dumpster pads. The yard hydraits have been
completed. Startup has been completed and it has been placed into
service.
e. Distribution Chamber
The reinforcing steel and the forms have been erected for the base
slab. Concrete has been placed for the base slab and North wall.
Approximately 60% of the reinforcing steel acid formwork has been
erected for the remaining walls of the distribiition chamber. Concrete
has been placed for the walls to elevation 92.17 feet The interior
walls have been prepped for waterproofing. The waterproofing has

been completed. The air piping has been set in place.. Startup has
been completed and it has been placed intoservice.
f. Electrical Building
Concrete has been placed for the columns. Approximately 70% of the
formwork; reinforcing steel and conduit has been installed for the
suspended slab. Concrete has been placed fo .the suspended slab and
the 8-inch masonry block walls have been erected. Approximately
70% of the brick veneer walls has been erected: The precast concrete
roof has been set in place. The remainder of the brick veneer has been
erected. The hollow core roof has been set in place. Additional .
electrical components have been set in place acid some wire has been
pulled. The remainder of the conduit has beefy completed and
approximately 60% of the wire has been pulled; The remainder of the
electrical panels has been set in place. The reiainder of the wire has
been pulled, startup has been completed and it has been placed into
service.
g.
Aeration Basin Improvements/Effluent Channel Construction
Sheeting has been driven adjacent to the existing effluent aeration
basin channel. The brackets have been installed and flowable-fill has
been injected to support the existing effluent aeration basin channel
(change order #3). Approximately 75% of the excavation has been
completed for the new effluent aeration basin channel. The remainder
of the excavation has been completed for the effluent channel:
Approximately 60% of the reinforcing steel aiid formwork has been
erected for the base slab of the proposed effhieiit channel between
aeration basins 3 and 4. The remainder of the 'reinforcing steel has
been erected and concrete has been placed for 50% of section- #1 base
slab between aeration basins 3 and 4. Concrete has also been placed
for 50% of section #1 walls. Concrete has been placed for the
remaining portion of section #1 base slab and Walls. Aeration basin #4
has been drained and approximately 40% of the forms and reinforcing
steel has been erected for the effluent channel ,wall inside aeration
basin #4. Concrete has been placed for the effitient channel walls
inside aeration basin #4. The diffusers have been replaced in aeration
basin #4 (change order #5). Aeration basin #4 has been placed back in
service. Concrete has been placed for the base slab of section #2
between aeration basins 3 and 4. Approximately 50% of the form and
reinforcing steel has been erected for the walls: Concrete has been
placed for the walls of section #2. Approximately 50% of aeration
basin #4 exterior wall at the effluent channel hats been removed. The
remainder of the aeration basin exterior wall (at the effluent channel)
has been cut and removed and the 10-inch and 6-inch header pipe has
been mounted. on the effluent channel wall. Aeration basin'#1 has -
been drained and the 30-inch slide gate has been set in place and
concrete has been placed for the influent box: Aeration basin #1 has
been placed back into service.: Aeration basin #2 has been drained and

the 30-inch slide gate has been set in place arid concrete has been
placed for the influent box. Concrete has beefi placed for the bases
for the piers of the air piping. Approximately 60% of the influent
structure has been completed and approximately 30% of the
demolition of the walls on the effluent end has been completed for
aeration basin #3. The remainder of the influent chamber box and
weir gate has been completed for aeration basitf #3. The walls and
adjacent base slab at the effluent end of aeration basin #3 has been
• demolished. Concrete has been placed for the Base slab at the effluent
end of aeration basin #3 and approximately 4t % of the reinforcing
steel and formwork has been erected for the Walls. The remainder of
the reinforcing. steel has been erected for the Wills (sections-3 & 4) of
the effluent channel at aeration basin #3 and eoncrete has been placed
for the inner walls. Concrete has been placed for, the outer wall of
sections 3. & 4. The bulkhead has been completed between aeration
basins #2 and #3.. Aeration basin #3 has been placed back in service
and aeration basin #2 has been taken out of seiice; The walls and
base slab have been demolished for aeration basin #2. The influent '
channel has been demolished for aeration basifis #1 and #2 and the
reinforcing steel and formwork has been erected for aeration basin #2.
Aeration basin #1 has been placed into service. with flow from the new
pump station and the new 36-inch PIE line. The effluent channel base
slabs have been completed for section 5 and seetion 6 and the walls
have been completed for section 5 and approximately 70% complete
for section 6 at aeration basin #2. The remainder of the wall has been
completed and the effluent channel has been placed in service at
aeration basin #2. The 36-inch PTE line has been installed at aeration
basin #2 and the excavation has been completed at aeration basin #3.
The wall cores have been completed for aeration basin #4 for the 16-
inch RAS line and the 30-inch PTE line. The 1 2-inch drain line has
been completed and backfill has been placed try an elevation of the
core for the 16-inch RAS line. The.wall cores have been completed
for aeration basin #5 and the effluent channel at aeration basins #2 and
#3 have been backfilled. The pipe wall penetrations have been
completed for the influent RAS piping at aeration basins #4 and #5. .
Approximately 20% of the demolition has been completed at aeration
basin #1 influent and effluent ends. The remainder of the demolition
has been completed at aeration basin #1. Corierete has been placed for
the base slab (sections #7 and #8) and approximately 10% of the
reinforcing steel and formwork has been erected for the walls at
aeration basin #1 effluent channel. Concrete has been placed for the
wall at aeration basin #4, however, the temporary bulkhead has not
been removed. Concrete has been placed for the effluent channel
Walls at aeration basin #1 and the temporary bulkhead has been
removed. The old effluent channel between aeration basins 3 and 4
has been backfilled and abandoned in place. Approximately 80% of
the handrail has been completed. The remainder of the handrail has
been completed and aeration basin #1 has been backfilled. The

temporary bulkhead has been removed at aeration basin #4. The RAS
lines have been put in operations at aeration basins 2, 3 and 5.
h. Chlorine Contact Basin
Approximately 50% of the excavation has been completed. The
temporary supports have been erected over the existing 60-inch
effluent line for the tie-in at the proposed influent and effluent line
locations: The groundwater elevation was higher than expected so the
excavation of this structure has been delayed bile to additional
groundwater pumping efforts. A new. plan has been devised to . -
dewater and place concrete for one section of base slab at a time
starting with slab #2. Excavation -for slab #2 has been completed and
subgrade stone has been placed. Concrete has been placed for base
slab #2. Concrete has been placed for a porticdn of wall #12. Concrete
has been placed for wall #5 and wall #13. The forms and reinforcing
steel have been erected for approximately 50% and 30% of wall #6,
respectively. The remainder of reinforcing steel and forms has been
erected and concrete has been placed for wall #6. Concrete has been
placed for 50% of slab #1. The interior forms. have been erected for
the remainder of wall #12 and a portion of wall #1 that is adjacent to
wall #12. Concrete has been placed for the retaining portion of slab
#1 and wall #3. Approximately 90% of the c ircrete has been placed
for walls: #1, #2, and #4. The remainder of the concrete has .been
placed for walls: #1, #2, and #4. Concrete was placed for base slab #4
and walls: #7 and #14. Concrete has been placed for the suspended
slab at the influent end of the basin. Concrete has been placed for slab
#5 and approximately 70% of walls #9, #10, c4 #11. Concrete has
been placed for the remainder of walls 9 thru 11 and the suspended
slab at the effluent end. The tank drain lines have been installed from
the sumps to the drain pump station and approximately 40% of the
backfill has been placed. "The bypass pumping has been completed
and demolition of the outfall line at the chlorine contract tank has been
completed and tied -in to the basin. Concrete has been placed for base
slabs #7 and #8 and walls #17 and #20. Concrete has been placed for
walls #15 and #18 and approximately 50% of the forms has been
erected for walls #16 and #19. Concrete has been placed for the
remainder of the walls (#16 and #19). The hydrostatic test has: been
complete for channels 2 and 3 and placement of backfill has started.
The drain pump station has been installed and the backfill has been
placed around the remaining perimeter of the structure. The startup
has been completed.
i. Maintenance Building
Concrete has been placed for the Maintenance building base slab. The
structural steel has been erected and the doorframes have been set.
The 8-inch masonry walls were approximately 50% complete. The
8-inch masonry-block/brick veneer walls were approximately 70%
complete. The remainder of the block/brick veneer walls has been

erected. The metal siding has been erected for the sides and roof and
approximately 30% of the insulation has beers placed on the interior
walls and roof. The remainder of the roof, dcdors and insulation has
been completed and approximately 30% of the painting has been
completed. The remainder of the painting has been completed and
approximately 50% of the HVAC has been set place. The
remainder of the HVAC has been completed, the floor has been sealed
and the building has been placed in service.
Aerobic Digesters and Blower
Concrete has been placed for approximately 91% of both digesters.
One of the digesters was ready to begin the hydrostatic test. The
hydrostatic 'tests have been completed for both digesters.
Approximately 75% of the under slab drainage has been completed.
The remainder of the under slab drainage has been completed and the
structure has been backfilled. The air distribtition piping has been
installed on the floor of digesters #4 and #5. 'the slide gates have been
installed in digester #4. Approximately 30% of the hand rail has been
installed. The slide gates have been installed Ili aeration basin #5.
Another 20% of the hand railing has been installed at digesters #4 and
#5. The bridge between. aerations #4-and #5 has been completed.
Concrete has been placed for the stairs and the air header support
columns. -Approximately 70% of the air header piping has been
installed. The air header .piping has been completed for digester #4.
The remainder of the stairs and hand railing has been completed. The
remainder of the air piping has been completed; wire terminations
have been completed, and startup has been completed. Aerated
digester #4 has been placed in service and the deliyery date for the
blower is two weeks (January 31, 2006). Blower #5 has been set in
place and approximately 80% of the blower piping has been installed.
Aerated sludge basin #5 has been placed in sei Vice. Approximately
90% of the blower #5 installation has been coiiipleted. Approximately
95% of the blower #5 installation has been cotiipleted. Approximately
50% of the fence has been erected around digesters #4 and #5. The
fence has been completed and tied to the existing fencing at both ends.
k. Yard .Piping
The 60-inch influent line from the influent jufiction box to the influent
pump station has been completed. Manhole 41 has been set in place
.at aerobic digester #5. Approximately 80% of the 12-inch sludge line
and 80% of the 4-inch scum line have been installed to digesters #4
and #5. The 36-inch PTE line from the distribution chamber to
aeration basin #1 has been installed. The remainder of the 12-inch
sludge line and the 4-inch scum line has been Completed at digesters
#4 and. #5. The 18-inch sludge drain line has been completed at
digesters #4 and #5. The remainder of the yard piping at digesters #4
and #5 has been completed and tested. Process drain MH #5 has been

installed and the 6-inch grit classifier line has been installed to MH #5.
The 18-inch process drain line and MH #2, #3; #4 and #6 have been
installed: The 2-inch non -potable line has been installed at the grit
facility and the influent pump station. The 20=inch RAS force main
has been, completed to the junction box. The M-inch PTE line has
been installed for aeration basin #2. The 20-ifit>h RAS force main has
been backfilled. The 36-inch PTE line has been completed for
aeration basin #3. The 30-inch PTE line has been completed for
aeration basin #4 and aeration basin #4 has been placed back in
service. The 30-inch RAS line has been installed from the Y-
connection to the tie-in adjacent to aeration basin #5 and from aeration
basin #5 to aeration basin #3. The force main from -the chlorine
contact tank pump station to the MH #3 has been completed. The 16-
inch RAS piping and the drainage piping have been set in place at
aeration basin #2 and #3. The remainder of the RAS piping has been
completed.
1. Electrical
The duct bank has been installed at the influent pump station. The -
conduit has been installed at the Electrical Building suspended slab.
The MCC has been received and set inside the Electrical Building.
Approximately 20% of the duct bank has been installed at the
digesters. The duct bank has been completed ffom the influent pump
station to the grit facility. Concrete has been placed for the primary
electrical service duct bank and transformer pad. The transformers
have been set at the influent pump station and the wire has been
pulled. Approximately 99% of the electrical has been completed at the
influent pump station/Electrical Building and fiber optics has been
completed to the Operations Building. The•femainder of the
.electrical has been completed at the influent ptifnp station and the
chlorine contact tank. Approximately 70% of the electrical work has
been completed for the controls at the aeration basins.
m. Demolition
Approximately 40% of the demolition of the influent pump station has
been completed. The remainder of the influent pump station has been
demolished and removed off site.
3. Contract 2 Cross Creek WWTP
Approximately 40% of the demolition has been; cdnipleted for influent
screw pump (ISP) #3. The remainder of the demolition has been
completed and the new ISP #3 has been set in plate. The contractor has
grouted ISP #3. The contractor has removed appf ximately 80% of the
grout for ISP #3 due to the grout prematurely setting up. The remainder of
the grout from ISP #3 has been removed: New Oita has been placed for
ISP #3 and approximately 95% of startup for ISP ##3 has been completed.
The remainder of the startup has been completed fair ISP #3 and ISP #2

has been demolished. Concrete has been placed fat the motor/gearbox pad
for ISP #2. ISP #2 has been set in place. The stai=tfzp has been completed
for ISP #2 and the demolition has been completed for ISP #1. ISP #1 has
been set (approximately 95 percent) in place. Startup has been completed
for ISP #1 and it has been placed back into service: The demolition of
clarifier #3 has been completed and work has begafi replacing RAS pump
station piping. The masonry wall at the grit facility has been completed.
The demolition has been completed for RAS purrips: #3, #4, and #5. The
miscellaneous metals at the receiving station have been completed. The
demolition of RAS pumps #1 and #2 have been cdthpleted and the piping
modifications have been completed for RAS pumps #1, #2, #3, #4, and #5.
The primary sludge pump #1 has been demolisher: The primary sludge
pump #1 has been installed and awaiting startup. Approximately .10% of
the formwork has been erected to shore up the latifider trough at clarifier
#3. The demolition of diffuser piping at aeration basin #3 has been
completed and approximately 90% of the RAS ptifhp station piping has
been painted. The grit removal pumps have been 'received. The diffused
aeration equipment in aeration basin #3 has been installed and startup has
been completed. Concrete has been placed for the launder trough at
clarifier #3. The air release valves have been installed at the anaerobic
digesters. Concrete has been placed for the stairs at the influent pump
station. The demolition of grit removal pump #1 has been completed and
approximately 90% of the installation of new grit femoval pump #1 has
been completed. The work at the thickened residual storage tank has been
completed. The motor/drive and bearings have been replaced for
intermediate ISP pump #1. The #3 primary sludge pump has been
completed and the air release valves have been set in place for the pumps.
Approximately 20% of the primer coating has beeti applied to the walls of
clarifier #3. The remainder of the coating system l'ias been applied to the
walls of clarifier #3 and approximately 80% of the clarifier equipment has
been completed. The remainder of the grit pumps and primary sludge
pumps has been installed. Approximately 80% of the storm drainage
modifications has been completed. The diffuser piping in aeration basin
#4 has been completed and the basin has been put back into service.
Intermediate ISP pumps #1 and #2 have been completed and placed in
service. Intermediate pump ISP #3 has been completed and placed in
service and the demo of intermediate pump. ISP #4 has been completed.
Primary sludge pump #2 has been completed and placed in service: The
storm sewer work has been set in place and it was determined that the soil
was pumping and may need to be replaced. Blower #1 has been set in
place and approximately 90% of the piping has been installed.
Approximately 95% of blower #1 installation has been completed.
Intermediate ISP pump #4 has been completed arid placed in service. The
demolition of ISP pump #5 has been completed. ISP #5 has been placed
back into service. The curb and gutter have been completed at the septage
receiving station. Clarifier #3 has been placed baler into service. Blower
#1 has been placed back into service. The subsurface stone has been
placed at the'$eptage receiving station. The pavifig has been completed at

4. Contract 2D
Concrete has been, placed for pads for generators (at Rockfish WRF and
Cross Creek WRF), their respective 5000-gallon fuel tank pads, and their
transformers pads. 'Concrete has also been placed for the pads of the
proposed Control Buildings at each generator site: The conduit between
the generator and the Control Building has been installed at both locations.
The generator, Control Building, control panel, and the fuel tank have
been set in.place at the Rockfish WRF and the Cross Creek WRF. Wire -
has been pulled between the generator, Control Building and transformer
at both generator sites. The terminations of the wire have been completed
for the Control Building and the transformer. The grounding grid has
been installed at the Cross Creek location. The grounding grid has been
installed at the Rock Fish location. The load bank tests have been
completed for both generators.
5. Contract 2E
The generators, have been delivered to the Rockfish WRF and the Cross
Creek WRF sites.
