Other Special Items: _ The Rockfish Water Reclamation Facility is presently a member of the Cape Fear River Assembly and instream Cape Fear River monitoring is performed by this group, thus upstream and downstream monitoring by the company should not be a permit requirement. PART IV - EVALUATIONIAND RECOMMENDATIONS EPA Form 1 was not included with FRO's permit application. This facility is in coinpliancewith allNPDES effluent permit .parameters. It is recommended that the Permit be issued.for 21.0 MGD flow:withthe appropriate limits for this completed desigmbeginning-upon receipt of the Engineer's: Certification for completion of the 21 MGD expansion and lasting until permit expiration. It is also recommended that monitoring limits not be included for further expansions at this time. The final effluent limits should be flow based on completed design not average flows. In the future, there will_be two (2) additional Significant Industrial Users: 1. DAK Americas, LLC (IUP# 2116RF and NPDES# NC0003719) that may send one-third (1/3) of its wastewater to PWC Rockfish WWTP. 2. NITTA Gelatin USA INC: This Significant Industrial User facility will extract gelatin from edible grade pigskins to create edible pharmaceutical grade gelatin. Black & Decker, Inc. Customer Service 1 800 623-7748 The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143-215.1 and in a manner approved by the North Carolina Division of Water Quality. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance c itions the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. The Fayetteville Regional Office, telephone number (910) 486-1541, shall be notified at. 'least forty-eight (48) hours in advance of operation of the installed facilities so that an on site inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional, engineer certifying that the permitted facility has been installed in accordance with the NPDES Permit, this Authorization to Construct and the approved plans and specifications. Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge .(ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each Class I facility at Ieast weekly and each Class II, III and IV facility at least daily, excluding weekends and holidays, must properly manage the facility, must document daily operation and maintenance of the facility, and must complywith all other conditions of T15A:8G.0202. A copy of the approved plans and specifications shall be maintained on file by the Permittee for the life of the facility. During the construction of the proposed additions/modifications, the permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at all times, and in such a manner, as necessary to comply with the effluent limits specified in the NPDES Permit. The inspection included the same objectives as that of a routine Compliance Evaluation Inspection plus an Aquatic Toxicity (AT) test. to evaluate the biological effect of the facility's discharge on test organisms. As part of the inspection, a tour of the Wastewater Treatment Plant was conducted. All observations and recommendations are in Part D, Summary of Findings/Comments of this. inspection report. :A whole effluent sample was collected at _Outfall . 001. from the 24-hour composite of May. 16 and May 19 for :use in a chronic Ceriodaphnia' dubia pass/fail-toxicity test. Thissample was sent to. the Division of Water Quality (AT) Laboratory, (located on Reedy Creek Road in Raleigh). The Whole Effluent Toxicity pass/fail test results using these samples resulted in a "Pass Test results for these samples. indicate that the effluent would not be predicted to have water quality impacts on receiving water. The plant effluent composite sample that was taken by DWQ-FRO at the Rockfish WRF Outfall 001- on May 16 were analyzed by the Division of Water .Quality Chemistry Laboratory, located on Reedy Creek Road in Raleigh; for 'the following parameters (which are currently in your NPDES Permit): Total Suspended Solids (TSS) = <5 mg/L, Ammonia Nitrogen =0.15 mg/L, Total Kjeldahl-Nitrogen (TKN) = 1.6 mg/L, Nitrates + Nitrites as Nitrogen =13 mg/L; and Total Phosphorus = 2.6 mg/L. a hCarolina NAaiura!!" In, addition, some of the temperatures exceeded the range of :1,,0 to 4.4 degrees Celsius. Requirements --- please take appropriate corrective actions concerning the inadequacies of the' following: #1) record the temperature of the effluent composite refrigerator each time that a composite sample is collected, #2) the acceptable temperature range of 1.0 to 4.4 degrees Celsius must not be exceeded. As a temporary measure, please place the composite sample container in an bath :inside of the' sample refrigerator; during collection, until the refrigerator. can be repaired or replaced. Please respond to this office by July 21, 2006 concerning the two above -mentioned requirements. Based upon the overall tour of the wastewater treatment plant, record keeping, self- monitoring/reporting, and maintenance, this facility appears . to be well maintained. The third clarifier -had been offline for approximately .three to four months; it is in working order.' There was aIarge amount•of duckweed in; this Clarifier. 'The temperatures of the effluent composite refrigerator were not being recorded adequately (please refer to the checklist "Effluent Sampling" for more details).
