The annual report refelcted compliance with the subject permit. Ms. Caudle briefly reviewed the annual report when submitted for completeness and notified Brent Collins of EMA Resources that a form was mislabled. Mr. Collines corrected the form and resubmitted the complete annual report. The following items of concern were noted onn the pathogen and vector attraction reduction (PVAR) forms for the water treatment plant and wastewater treatment plant. * The vector attraction reduction section of the PVAR form shows that a 30 day bench scale test was use to determine compliance with vector reduction standards, but the lab reports provided in the annual report are for volatile solids reduction. * According to the fecal results provided in the annual report for the wastewater treatment plant, the values reported on the PVAR form are incorrect. The fecal values on the form range from 1635 to 3105 CFU, and the lab reports provided range fror 10,197 to 21,226 CFU. *According to the fecal lab reports provided in the annual report for the water treatment plant, the values reported on the PVAR form are incorrect. The fecal values on the form range from 193 to 198 CFU, and the results provided show the CFU values were all below reporting limits. It is not possible to know for certain the fecal values below the reporting limit. Based on the lab reports provided in the annual report, the facility was compliant with sampling frequency and limits. It appears that the facility updated the date on the 2019 forms and did not update the fecal values or vector reduction analysis.
