3. Project Description
3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application: The existing conditions onsite include a pump station, a gravel access road and an old, abandoned sediment
basin. It is our understanding that the abandoned sediment basin onsite was utilized during the construction of the
adjacent residential development. The general land use in the vicinity of the project is residential.

A wetland delineation was not completed on this site.

Approximately 131 linear feet of stream exists onsite.
3d. Explain the purpose of the proposed project:
The purpose of the proposed project is to repair and stabilize a stream that once existed in the footprint of an abandoned
sediment basin and remove an unsafe CMP riser basin structure that is a safety hazard.

The overall project consists of removing the existing CMP riser structure, rock and sediment of the abat -,, oned
sediment basin; and to restore and stabilize the stream by utilizing cross vanes and boulder sills. In addition, the
stream banks and riparian area will be planted with trees, shrubs and live stakes. Equipment typically used for
restoration projects will be utilized.

D. Impact Justification and Mitigation
1. Avoidance and Minimization
1 a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project.
It is our understanding that a sediment basin was created during the construction phase of an adjacent residential
subdivision. Currently, within the footprint of the old basin, an unstable, over - widened stream and riser barrel
exists. The Town of Kernersville would like to repair and stabilize the stream system. This repair and stabilization
would benefit this system by minimizing the sediment going downstream and restoring a natural stream system.
Temporary Erosion & Control measures will be utilized during construction activities to allow for a dry work area,
these temporary measures consist of a coffer dam and gravel check dam which will total 20 linear feet of temporary
impacts. In addition, a pump around system will be utilized to maximize the dry work area. Permanent structures
(i.e. boulder cross vanes with step, boulder sills, log sills, log J -hooks and boulder J- hooks) will be constructed to tie
in the elevation from the existing road culvert to the downstream side of the riser barrel and pipe structure which is a
total of 6% slope. It is anticipated that approximately 117 linear feet of stream channel will be restored post
construction, along with a riparian area planted with shrubs and trees.
Furthermore, the applicant is aware of the additional regional conditions associated with the Nationwide Permits 27.
With respect to utility line activities (NWP 27- Additional Regional Conditions section 4.0) this project will not be
supplying mitigation credits to the NCEEP.

We have reviewed the "Draft Internal Policy, Cumulative impacts and'the 401 Water Quality Certification and Isolated
