1. Mr. Tony Parker of the Mooresville Regional Office of the NC Division of Water
Resources (DWR or the Division) conducted a compliance evaluation inspection (CEI) of
the subject wastewater collection system (WWCS) on July 24, 2019. The assistance you
provided during the inspection was greatly appreciated. A copy of the inspection report is
attached for your records and inspection findings are summarized below.
Site/System Review
2. The following system components were inspected:
a. Various manholes were inspected. One issue noted. Please see attached report.
b. Two pump stations were inspected.
c. One aerial crossing was inspected.
3. As you are aware, owners of permitted collection systems must comply with their permit
and all rules listed under NC Administrative Code 15A NCAC 02T .0400, et seq. The
system status is outlined below:
a. 15A NCAC 02T .0403(a)(1): Requires that the system be effectively maintained
and operated at all times to prevent discharge to land or surface waters and any
contravention of ground or surface water standards.
Observations: The system has had no reportable spills and appears to be well
maintained and operated.

b. 15A NCAC 02T .0403(a)(2): Requires that a map of the wastewater collection
system be developed and actively maintained. -
Observations: A detailed map was available.
Compliance Status: Compliant
c. 15A NCAC 02T .0403(a)(5): Requires that the permittee or its representative
inspect high -priority sewers (HPS - as defined in 15A NCAC 02T .0402(2)) at least
once every six months and document the inspections.
Observations: The system has one High Priority line or aerial crossing which is
in the process of being replaced.
Compliance Status: Compliant
d. 15A NCAC 02T .0403(a)(6): Requires that the permittee or its representative
conduct a general observation of the entire wastewater collection system at least
once per year.
Observations: The collection system has approximately 11,880 feet of gravity
main R/W and is viewed/mowed and documented as required. In addition,
approximately half of this line is under construction contract and is being replaced.
Compliance Status: Compliant.
e. 15A NCAC 02T .0403(a)(7): Requires that overflows and bypasses from the
system be reported to the Division's Mooresville Regional Office in accordance
with 15A NCAC 02B .0506(a), and that public notice must be provided as required
by G.S. 143-215.1C.
Observations: The system has not had any reportable spills for this review period.
The NC regulations and statutes, which cover the reporting requirements for SSOs,
are found in NC Administrative Code 15A NCAC 02B .0506(a) and NC General
Statute 143-215.1C, respectively. You may access the NC regulations online at the

When reporting SSOs to DWQ, you must call the Mooresville Regional Office
(MRO) at 704-663-1699 during normal business hours. If outside normal business
hours, you must report the spill to the NC Division of Emergency Management at
1-800-858-0368. Please note that all reportable SSOs must be reported within 24
hours of the occurrence, or first knowledge, of the SSO. Please also note that voice
mail or faxed messages are not considered as the initial verbal report. You must
talk to and report the spill to a live person.
Compliance Status: Compliant
f. 15A NCAC 02T .0403(a)(8): Requires that a grease control program be developed
and implemented as follows:

1. For privately owned collection systems, the Grease Control Program shall
include at least bi-annual distribution of grease education materials by the
owner or representative.
Observations: Educational information is posted at Town Hall and hand delivered
to all 182 customers.
15A NCAC 02T .0403(a)(10): Requires that documentation be kept for all
activities you undertake to comply with the requirements of 15A NCAC 02T .0403,
subparagraphs (a)(1) through (a)(9), for a minimum of three years, with the
exception of the map, which shall be maintained for the life of the system. Also any
new additions, subdivisions or phased development needs to be incorporated as
well.
Observations: Records are maintained as required and a system map was available.
Compliance Status: Compliant

Project Description: The proposed project includes sewer system rehabilitation in the existing
High Shoals collection system and the extension of the High Shoals wastewater collection
system to a new Gaston County sewer system to convey wastewater flows to the City of
Gastonia's Long Creek WWTP. The proposed project also includes decommissioning two
existing wastewater treatment plants in the City of High Shoals.
Improvements to the High Shoals collection system will include cleaning and video inspection of
the existing 10,600 linear feet of existing sewer and proposed sewer system rehabilitation
consisting of approximately 2,500 linear feet (1.f.) of sewer line to be replaced, along with
approximately 20 manholes. The High Shoals sewer system extension includes two (2) pump
stations, approximately 1,1001.f. of 4-inch force main, approximately 13,960 1.f. of 8-inch
gravity sewer, and approximately 1,8601.f. of 12-inch gravity sewer.
The High Shoals sewer extension will connect to a new Gaston County sewer system which
includes four (4) lift stations, approximately 2,775 1.f. of 4-inch force main, approximately 5,158
1.f. of 8-inch gravity sewer, approximately 4,181 1.f. of 12-inch gravity sewer, and 25,902 1.f. of
15-inch sewer. The County system will also include a connection to the Gaston County landfill
and approximately 12,33911 of 10-inch force main from the proposed Dallas -Stanley Highway
pump station to convey flows to the City of Gastonia's Long Creek WWTP.
The City of High Shoals has previously been awarded assistance from the State Grant program to
complete improvements to their wastewater facilities. Gaston County intends to apply for
assistance from the State Revolving Fund program to construct the county facilities described
above, thereby providing an interconnection between High Shoals and Gastonia. The North
Carolina Division of Water Quality (DWQ) has conducted a review of the project in accordance
with the procedures (NCAC 15A Subchapter 1 C) for conformance with the North Carolina
Environmental Policy Act. The DWQ has determined that this project is a minor construction
activity, and that the preparation of additional environmental documents is not required.

This letter is to inform you of the Division of Water Quality's new policy concerning sewer collection
issues. In the attached Press Release you will note the use of the word "will" in regards to issuing
Notices of Violation (NOVs) for Sanitary Sewer Overflows (SSOs). The intent of that statement was to
make sanitary sewer system owners and operators aware that each SSO will be evaluated on a case -by -
case basis to determine if a NOV is warranted. The statement was not intended to indicate that in all
cases an NOV will be issued without a review of case specific factors.
You may have also noted a second statement using the word "will" regarding the issuance of civil
penalties. Civil penalties are only issued based on case -specific factors that include, but are not limited
to the following: volume, volume reaching surface waters, duration, gravity, impacts to public health,
fish kills or recreational area closures. Again, the statement is not intended to indicate that in all cases a
civil penalty assessment will be issued without review of case specific factors.
In addition, the Division is requiring a Flow Tracking/Acceptance for Sewer Extension Permit
Applications Form (FTSE-10/07). A revised form is enclosed. Effective January 15, 2008, this form
must be completed by the owner of the receiving WWTP and submitted with all applications for sewer
line extensions. Additional forms may be required if entities other than the owner of the WWTP own
pump stations along the route of the proposed wastewater flow. These entities would certify and submit
the additional forms. Applications received after the implementation date that fail to include this
information will be returned immediately as incomplete.
Section I, (d) of the FTSE-10/07 requires an estimate of the obligated flow not yet tributary to the
WWTP. Reviewing flow allocations made over the last two years, WWTP owners must reconcile their
flow records, to the best of their ability, so it is known how much flow has been obligated, but not yet
made tributary to the WWTP. Submit FTSE-10/07 to the Mooresville Regional Office with Section I,
(a) — (e) completed by January 15, 2008. Please note that items (a) — (h) in Section I, will be completed
for fizture sewer extension applications.

Resources (DWR) conducted a compliance evaluation inspection for the City of High Shoals (the
City) wastewater collection system on August 12, 2013. Your assistance was greatly
appreciated. An inspection checklist is attached for your records and inspection findings are
summarized in this letter.
System Review
Two new pump stations were placed in service in Spring of 2013 for the City of High
Shoals under the authority of Permit No. WQ0035209. In addition, an interconnect was made
between the City and Gastonia's Long Creek WWTP which included additional pump stations
that were permitted to Gaston County as the owner. If the County is not the owner and operator
of the system, please submit the name change form included with this letter.
Permit No. WQ0035906 was issued to repair and replace lines in the City that were known to
have a high inflow and infiltration rate during wet weather. This permit was issued in the City of
High Shoals.

Owners of deemed permitted collection systems must comply with all rules listed under
NCAC 15A NCAC 02T.0400, a copy of these rules is included with this letter. Documentation
of the inspections for both pump stations and high priority lines is required. Overflows are to be
reported to the Regional Office within 24 hours of occurring and 5-day reports are to follow for
completed information. A copy of this report is also included with this letter.
A grease control program is required for publicly owned systems. A bi-annual
distribution of grease education materials for both commercial and residential users is required.
Documentation of the distribution of the material should be made as well as any other effort to
educate the system users about grease.
Right-of-ways and easements are to be maintained for accessibility of equipment needed
to maintain the collection system.
All documentation for all activities to comply with the requirements should be kept for a
minimum of three years, and the collection system map should be maintained for the life of the
