Description of projects area and activity: The purpose of the project is to stabilize 530LF of stream bank erosion & establish a woody buffer The project involves the installation of a j-hook, bankfull bench, and single -wing deflector, as well as sloping and planting. Work will be performed from the bank with the exception of the jhook installation, when heavy equipment will be used within the channel. If Eastern Hellbenders are disturbed during construction. thev should be cautured in a bucket and moved away from the area of disturbance. Contact Lori Williams (lori.williamsnancwildlife.org) if either Appalachian Elktoe or Eastern Hellbenders are seen. 2. Due to the sensitivity of local species to fine sediments, it is essential that excellent erosion and sediment control be maintained during the project. 3. Work should be performed during low flows, and a coffer dam should be used to divert flows from instream work areas at the j-hook and single -wing deflector locations. 4. The designer or their designee shall be present and supervise the installation of in -stream structures. 5. Visual monitoring of the stabilization project shall be conducted at a minimum of quarterly for the first year or two bankfull events (whichever is longer), and then annually until the site is stable. If further slope failure occurs, you will need to contact USACE to determine further permitting options. Your work is authorized by the above referenced permit provided it is accomplished in strict accordance with the attached conditions, above noted special conditions, and your submitted application dated 11/10/2020. Any violation of the attached conditions or deviation from your submitted plans may subject the permittee to a stop work order, a restoration order, a Class I administrative penalty, and/or appropriate legal action. This verification will remain valid until the expiration date identified below unless the nationwide/regional authorization is modified, suspended orrevoked. If, prior to the expiration date identifiedbelow, the nationwide/regional permit authorization is reissued and/or modified, this verification will remain valid until the expiration date identified below, provided it complies with all requirements ofthemodifiednafionwide/regionalpermit. Ifthenationwide/regionalpermit authorization expires or is suspended, revoked, or is modified, such that the activity would no longer comply with the terms and conditions of the nationwide/regionalpermit, activities which have commenced (i.e., are under construction) or are under contract to commence in reliance upon the nationwide/regional permit, will remain authorizedprovided the activity is completed within twelve months of the date ofthe nationwide/regionalpermit's expiration, modification orrevocation, unless discretionary authority has been exercised on a case -by -case basis to modify, suspend orrevoke the authorization. Activities subject to Section404 (as indicated above) may also require an individual Section401 Water Quality Certification.
