My apologies for taking so long to get back to you on this subject.
It appears that the file for Axalta was missed when files were initially being uploaded to Laserfiche. I am currently
working on scanning the physical file and getting it uploaded.
DEMLR has the Change of Ownership form and the letter that approves the change of ownership. Axalta is currently
covered under an active No Exposure Certification (NCGNE0758).
My understanding is that the rescission was requested by Engineered Polymer Solutions in 2017, due to the ownership
change, but was denied due to the unchanging operations at the facility. This prompted the need of a Change in
Ownership form instead of a rescission request.
Our records are now up to date and no additional actions are required by Axalta at this time concerning this matter. I
apologize for any confusion this may have caused.

Hi Michelle,
I was really hoping to get a copy of the actual certificate for our audit records since I can't seem to locate one here.
Would there be an archive copy you could access?
I was able to locate a name change to Axalta from 9-2017 as well as the original request from 6-2017. But there was
also, apparently, an attempt to rescind the permit which was denied in 6-2017 which I can't really make sense of. I've
attached those to see if they are any help in locating the actual certification document.
