Karyn, thanks so much for the thorough information. Yes, that close-out document should be sufficient to terminate the
COC in question. Alaina Morman on our staff will work with the permittee through that NOT review process. We will
also link these other COC files together in our repository so this information is easier to track. Please contact Alaina if
you have any other questions about the termination and resolving the annual fee for this one. She will be in touch if she
has any questions.

It does appear that we have double permit coverage, from what I was able to find. Please see below:
Permit in question: SEC-030184-2019/NCC200659 — covers Willoughby Lots 33-36, 40 & 41
Willoughby Lot 33 — also covered under SEC-051967-2021/NCC210922
Willoughby Lot 34 — also covered under SEC-044415-2020/NCC202870
Willoughby Lot 35 — was not able to find additional E&SC plan approval nor certificate of coverage
Willoughby Lot 36 — also covered under SEC-033466-2020/NCC200928 (I think that this was a modification)
Willoughby Lot 40 — also covered under SEC-029709-2020/NCC193346
Willoughby Lot 41— also covered under SEC-035645-2020/NCC201624
On 11-30-20, 1 closed lots 35, 36, 40, and 41 under the Permit in question: SEC-030184-2019/NCC200659 —which covers
Willoughby Lots 33-36, 40 & 41. Now that the error has been found, let me know if you also need for me to issue new
Certificates of Completion to address the additional Certificates of Coverages on lots 36, 40, and 41 (last inspection in
early February found lots 33 and 34 were still active)? Also, with this additional information and with Drees completing
the Notice of Termination promptly, would you be able to accept the attached Certificate of Completion to terminate
coverage for SEC-030184-2019/NCC200659?
Linda or Shawn, can you give NCDEQ an update on how soon that you could have the eNOT form completed in the
NCDEQ Portal?

This Certificate of Coverage was issued on 2/19/2020. If the project was not completed and closed -out before a year,
the $100 fee applies. However, if a close-out request prior to that date (we see you sent the request to Raleigh on 2/1)
did not result in an inspection and ability to submit a notice of termination by the invoice due date (3/1/2021), please
contact our Program supervisor Annette Lucas for further direction on fee status.

You will need to work with Karyn Pageau or Jeevan Neupane to close out this grading permit, SEC-
030184-2019, for Willoughby Lots 33-36, 40, 41. If the lots are not stabilized and the permit is not ready
to be closed, you may have to pay the renewal fee of $100 to NCDEQ.
