The Division of Water Quality (Division) Winston Salem Regional Office has reviewed the recent monitoring results specific to the
continued monitoring as noted in a letter by the Division on July 21 2011 for the above referenced site In addition a site visit was
conducted on June 13 2012 with you David Brame and Michael Brame of ECS Carolinas and John Thomas with the USACE The
Division is in receipt of a letter dated June 14 2012 from ECS Carolinas requesting complete termination of the compliance
monitoring program for this site The Division concurs with this request and has determined that the Horsepen Creek Mitigation Site
has met the established success criteria at this time
The long term protection mechanisms included in the approved Mitigation Plan must remain in effect in perpetuity
