USA, Permit No. CM-33 Was under a Schedule of Compliance from February 25th 2020 to October 20th 2020(see SOC copy enclosed). The termination of the compliance schedule was based on the removal of an IPA disinfectant and the submitted study (Operational Excellence Metrics Summary Report). The permit was modified and approved on October 20th to include daily max concentration and monthly averages for the CFR 439 organic compounds. They were over the monthly average for acetone in October,which resulted them being SNC for the second period of 2020. The facility has been listed in a public notice in The Enquirer Journal and was assessed a civil penalty of$500.00 ELG Utica Alloys Permit No. CM-34 The facility caught fire on April 7th 2020 and damaged the pretreatment system and processing area. No process wastewater has been discharged since. They requested on January 7th 2021 to have their permit canceled, a closure inspection has been performed and documentation will be sent in another packet. NC Biogas Permit No. CM-35 Construction for the project was delayed due to Covid-19, construction is expected to begin 2021. There was zero discharge for the year of 2020. Dental Amalgam Rule: To comply with 40CFR part 441 a total of twenty-one forms were sent and twenty-one returned, including four from new sources. Permits were renewed/modified and approved for ATI Allvac Monroe CM-15, ATI Specialty Materials-Bakers South Operations CM-30, ELG Utica Alloys CM-34, GM Nameplate CM-20, Glenmark Pharmaceuticals, Inc. USA CM-33,NC Biogas CM-35 and Tyson Farms CM-11. Complete wastewater testing/characterization and engineering design, by Junel, 2020. b. Submit pretreatment improvement plans and request for Authorization to Construct by June 15,2020. c. Complete and equipment bidding/funding,and vendor selection by July 6,2020. d. Complete pretreatment system installation,testing, and commissioning by March 1, 2021. 5. Glenmark shall submit progress reports to the City within 14 days of the completion of milestones shown above. 6. To assist Glenmark in completing the corrective action plan and to comply with all conditions of their permit,this Schedule of Compliance is hereby established and will remain in effect from the execution date until March 1,2021.
