I am aware that there are significant penalties for submitting false information. retreatment Coordinator D off. a,S/a at TOWN OF YADKINVILLE NPDES # NC0020338 2020 PAR NARRATIVE GENERAL INFORMATION: PROGRAM INFORMATION: • AT, LTMP, SUO, ERP, IWS, HWA and Permits are up to date. • New NPDES Permit was received 1/28/2020. Effective date 3/1/2020. • A 5-year compliance schedule for copper was included within the new Permit. • An updated LTMP was submitted on 6/15/20 and approved on 9/14/2020. • An updated IWS was submitted on 1/6/2017 and approved 4/7/2017. • An updated HWA was submitted on 8/18/2017 and approved on 10/31/2017. • A Pretreatment Compliance Inspection (PCI) was performed on 11/05/2020 by Jim Gonsiewski from the WSRO. • Approval date of the SUO was corrected on the Pretreatment Program Database from 01/02/2012 to 01/03/2012 as a corrective action after the PCI. • Civil Penalties collected for 2020 were $172.60. • Yadkinville WWTP has 3 Dental facilities discharging into the POTW. All 3 facilities were notified on 6/30/2020 with certified letters with the required forms and instructions to be filled out. Two of the three (67%) dentists have completed and returned their required forms. The dentist that has not returned her forms, contacted me on 10/30/2020 via phone and requested a second copy of the forms. The forms were emailed on 10/30/2020 and I haven't received any response as of 2/25/2021. PERMIT INFORMATION THE AUSTIN COMPANY IUP # 0006 - Metal Finisher SNC INFORMATION: • SIU was in SNC for the 1st 6 months period of 2020 for Cyanide. Austin is required to collect 1 sample/ 6 months and the POTW collects 1 sample/ 6 months. The sample Austin collected was compliant however the sample the POTW collected exceeded Austin Company's limit. This made them 50% SNC. Austin company was placed in a Public Notice and issued an NOV and issued the cost of publication. Upon further inspection of all cyanide data collected for SIU's in April 2020 it was determined that a lab error was the most likely cause of the elevated cyanide result for Austin. The POTW collected an additional cyanide sample which was compliant. ORDERS AND SCHEDULE INFORMATION: None A TO C AND CONSTRUCTION INFORMATION: None MISCELLANEOUS INFORMATION: • TTO Certificates statements were received for both halves of 2020. • Austin Co. IUP renewed 4/1/2019 The Austin Co. had the following violation(s) for the January to June reporting period: • 1st 6 months- SNC for Cyanide NOV issued and cost of Publication. The Austin Co. had the following violation(s) for the July to December reporting period: • September 2020- Late reporting NOV issued and $100.00 fine issued. NOVs were sent for all violations. The Austin Co. was inspected by the Town's Pretreatment Department on 7/17/20. Flow meter calibrations were performed as required by permit for 2020. B&G Foods, Snacks IUP # 0004 — Bakery SNC INFORMATION: • SIU was not in SNC this PAR year (2020) ORDERS AND SCHEDULE INFORMATION: A TO C AND CONSTRUCTION INFORMATION: None. MISCELLANEOUS INFORMATION: B&G Foods Snacks, Inc. Was issued a new permit on 6/12/2015 Expiration of current permit is 5/30/20. Application for permit renewal on file at WWTP. Permit issued 5/12/2020 effective date 6/1/2020. B&G Foods, Snacks had the following violation(s) for the January to June reporting period: • February 2020- NOV issued for missing 1- Cd, Pb, Ni sample for the 1st 6 month period. PT Department noticed during PAR data entry. NOV to be issued 2/2020 and carried over to 2020 PAR. Missed issuance of 1 Oil & Grease NOV. • May 2020- NOV issued for 1 flow violation $0.00 fine issued. B&G Foods, Snacks had the following violations for the July to December reporting period: • July- NOV issued for non -reporting of 4 flows, temperatures and pH's. • August- NOV issued for 2 flow violations. B&G Foods, Snacks was inspected by the Town's Pretreatment Department on 11/12/2020. Flow meter calibrations were performed as required by permit for 2020. UNIFI, INC. IUP # 0002 — Textile SNC INFORMATION: None ORDERS AND SCHEDULE INFORMATION: None. A TO C AND CONSTRUCTION INFORMATION: None. MISCELLANEOUS INFORMATION: Permit renewed 4/2/2019 and modified 11/6/2019. Modification was to lower pH limit from 6.0 S.U. to 5.5 S.U. Unifi IUP 0002 continues to have issues with monitoring equipment at their sampling manhole since it was run over by a semi truck in 2016. Unifi personnel have stayed in contact with me and informed me of the progress. Unifi has moved their sampling manhole and equipment to a new permanent location upstream 30-35 foot. This action was taken to potentially prevent future damage/failure to sampling equipment. Unifi, Inc had the following violations for the January to June reporting period: • January 2020- NOV issued for 8 flows missing due to damage from traffic. No fine issued. • February 2020- NOV issued for 4-pH violations, 3-Temperature violations. • April 2020- NOV issued for 2-flow violations and 1-(TRC)Cyanide violation. Unifi, Inc. had the following violations for the July to December reporting period: • July 2020- NOV issued for 6-pH violations. • September 2020- NOV issued for 2-pH violations. • 2nd 6 months of 2020- Unifi collected phenol samples but contract lab did not pick up the samples. NOV was issued February 2021 and will be included in 2021 PAR. NOVs were sent for all violations. Unifi, Inc. was inspected by the Town's Pretreatment Department on 12/04/20. Flow meter calibrations were performed as required by permit. UNIFI, INC. IUP # 0005 — Textile SNC INFORMATION • SIU was not in SNC this PAR year. Unifi, Inc. had the following violation(s) for the July to December reporting period: • October 2020- NOV issued for 1-flow violation. Discovered during PAR preparation due to circumstances, the PT team being under COVID quarantine a copper (TRC) violation wasn't discovered until post quarantine. NOV will be issued and included in 2021 PAR. NOVs were sent for all violations. Unifi, Inc. was inspected by the Town's Pretreatment Department on 12/04/20. Flow meter calibrations were performed as required by permit.
