I just received the revised plans in my office and I am ready to approve them However I was unable to locate an
Operation and Maintenance (0 &M) Agreement for the device which is required If you haven t already prepared one
then you can use the Sample 0 &M agreement for level spreaders available on our web site at
http //portal ncdenr org /web /wq /ws /su /bmp ch8
I have attached a copy of the document in MS Word so that you can edit it easily The items pertaining to the level lip
will not pertain to your device you may wish to replace those with more appropriate measures As soon as we receive a
copy of the signed and notarized 0 &M agreement in this office we can approve the project
