Closed Loop Recycled Wastewater Permit No. WQ0000962 Guilford County On February 11, 2021 Division of Water Resources staff person Caitlin Caudle completed a scheduled compliance inspection. A review of records, facilities, and monitoring wells was completed. Mark Creech (Plant Manager), Chris Miles (Safety and Environmental Manager), Derr Leonhardt (Environmental Consultant), Tyler Miles (Environmental Consultant), and Steve Grice (Human Resources Manager) of Culp Home Fashion were present during the review of records. Barry Bryant (Operator) joined the inspection during the review of the facility and monitoring wells. The facility was mostly compliant with the subject permit. The following items of concern were noted: • Per Permit Condition 11.3 "groundwater monitoring wells shall be constructed in accordance with 15A NCAC 02C .0108." Regulation 15A NCAC 02C .0108(1) requires non -water supply wells be "secured with a locking well cap to ensure against unauthorized access and use," and 15A NCAC 02C .0108(m) requires "wells shall be equipped with a steel outer well casing or flush -mount cover, set in concrete, and other measures to protect the well from damage by normal site activities." At the time of inspection, the cap on MW4 was rusted off at the hinge. Please make the appropriate repairs to replace the cap on MW4. The well casing on MW5 could not be closed due to the well casing settling. The concrete footing was also not visible. Please uncover the concrete footing of MW5 to ensure it is not damaged and complete repairs if needed; and make the necessary repairs to the well casing to properly close and lock the cap. If the well casing is replaced, please ensure all 15A NCAC .0108 regulations are followed. D_E NOR! Moving forward, please ensure all well caps are secured. • Per Permit Condition 111.7 "a protective vegetative cover shall be established and maintained on all berms, pipe runs, erosion control areas, surface water diversions, and earthen embankments..." At the time of inspection, the earthen berm between the concrete retention basin and Bear Creek did not have a well - established vegetative cover. When possible, please establish a vegetative cover on the berm and the area surrounding the concrete retention basin. • The soil below the pipe draining the area between the concrete retention basin and the earthen berm is beginning to erode, and can cause future structural problems to the berm. Please place riprap, or other material, below the pipe to disperse the stormwater that is drained from the area. • As mentioned in correspondence dated January 12, 2018 and February 8, 2019 the facility continues to be non -compliant concerning the use of the earthen berm as part of the wastewater treatment system. There is visual evidence that the berm is capturing treated mist that is drifting from the evaporator, with the effluent eventually being discharged to Bear Creek comingled with rainwater. As part of permit renewal, the Division plans to investigate the nature of treated effluent being released by the evaporator and its impacts to the surrounding area and surface waters. Please refer to the enclosed inspection report for additional comments. A follow-up inspection will be scheduled in 90 — 120 days to review repairs.
