This caused sewage to flow into a storm drain and then into Lake Norman. Repairs were made and normal operations were established at 5:35pm on May 29, 2020. The incident was reported to NCDENR. Spill Amount: 600 gallons Impact: None detected. Corrective Action: Ground area excess spillage was vacuumed. 2. Date: November 4, 2020 Time: 9:10 am Location: 7787 Water Oaks Dr. Denver, North Carolina Incident: NCDOT hit a 2.5" low pressure sewer line while repairing a culvert. This caused sewage to flow into an unnamed tributary of Lake Norman. Repairs were made and normal operations were established at 9:17am on November 4, 2020. Incident was reported to NCDENR. Spill Amount: 150 gallons Impact: None detected. Performance: Killian Creek WWTP operated in compliance with our NPDES permit during the calendar year of 2020. Compliance was reported on monthly Discharge Monitoring Reports submitted to the North Carolina Division of Water Quality during the year Killian Creek WWTP had the following reported Notice of Violations (NOV) 1) March 315t the NH-n3 Ammonia was out of limits causing the WWTP to be out of compliance for the month of March, 2020. No civil penalty issued. 2) April 4th and 30th the NH-n3 Ammonia was out of limits causing the WWTP to be out of compliance for the month of April, 2020. Civil penalty was issued. 3). May 16th and 30th the NH-n3 Ammonia was out of limits causing the WWTP to be out of compliance for the month of May, 2020. No civil penalty issued. III. II. Performance: The Lincoln County Sewer Collections System had the following reportable sanitary sewer overflows (SSO's): 1. Date: September 10, 2019 Time: 09.33 am Location: Lift Station # 12 2460 Hwy 16 N Denver, North Carolina Incident: A pump malfunction, electrically shorted several wires and tripped the control breaker, which resulted in the loss of power on the 110 volt circuit for control power. The auto dialer was connected to the 110 volt circuit and failed to identify the problem. This caused sewage to flow into a ditch next to the Lift Station. Repairs were made and normal operations were established at 10:15 am on September 10, 2018. Incident was reported to NCDENR. Spill Amount: 3,000 gallons Impact: None detected. This modification is to change ownership from Hecht Properties, LLC to the East Lincoln County Water and Sewer District, and East Lincoln County Water & Sewer District effective April 6, 2015, is now responsible for the continued operation and maintenance of the subject wastewater collection extension. Our database has been revised to reflect the ownership change. This approval letter shall be considered part of the permit and is therefore incorporated by reference. Other than ownership transfer, this modification includes no changes to the originally issued permit issued July 27th, 2005 and is subject to the conditions and limitations specified therein. If an individual permit is not required, the following performance criteria shall be met as provided in 15A NCAC 2H .0227: a. The sewer system shall be effectively maintained and operated at all times to prevent discharge to land or surface waters, and any contravention of the groundwater standards in 15A NCAC 2L .0200 or the surface water standards in 15A NCAC 2B .0200. b. A map of the sewer system shall be developed and shall be actively maintained. c. An operation and maintenance plan shall be developed and implemented. d. Pump stations that are not connected to a telemetry system shall be inspected every day (i.e. 365 days per year). Pump stations that are connected to a telemetry system shall be inspected at least once per week. e. High -priority sewer lines shall be inspected at least once per every six-month period of time. f. A general observation of the entire sewer system shall be conducted at least once per year. g. . Inspection and maintenance records shall be maintained for a period of at least three years. h. Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with 15A NCAC 2B .0506(a), and public notice shall be provided as required by North Carolina General Statute §143-215.1C. 4. This permit shall not be transferable. In the event there is a desire for the wastewater collection facilities to change ownership, or there is a name change of the Permittee, a formal permit request shall be submitted to the Division accompanied by documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request shall be considered on its merits and may or may not be approved. 5. Construction of the gravity sewers, pump stations, and force mains shall be scheduled so as not to interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of the State. 6. Per 15A NCAC 2H .0220, upon completion of construction and prior to operation of these permitted facilities, the completed Engineering Certification form attached to this permit shall be submitted with the required supporting documents to the address provided on the form. A complete certification is one where the form is fully executed and the supporting documents are provided as applicable. 7. A copy of the construction record drawings shall be maintained on file by the Permittee for the life of the wastewater collection facilities. 8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H .0200; the Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump Station and Force Mains adopted June 1, 2000 as applicable; and other supporting materials may subject the Permittee to an enforcement action by the Division, in accordance with North Carolina General Statutes §143- 215.6A through §143-215.6C. 9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement facilities. 10. The issuance of this permit shall not exempt the Permittee from complying with any and all statutes, rules, regulations, or ordinances that may be imposed by other government agencies (local, state and federal) which have jurisdiction, including but not limited to applicable river buffer rules in 15A NCAC 2B .0200, erosion and sedimentation control requirements in 15A NCAC Ch. 4 and under the Division's General Permit NCG010000, and any requirements pertaining to wetlands under 15A NCAC 2B .0200 and 15A NCAC 2H .0500. 11. II. Performance: The Lincoln County Sewer Collections System had the following reportable sanitary sewer overflows (SSO's): 1. Date: May 27, 2014 Time: 09:00 am Location: 7533 South Little Egypt Road Stanley, North Carolina Incident: Severe lightning strike to the SCADA and Pump Control Panel causing both pumps to fail and the SCADA System to fail to report a pump failure. Resulted in the pump station sewer backing -up and overflow an adjacent sewer manhole into the Forney Creek. Repairs were made and normal operations were established at 10:30 am on May 27, 2014. Incident was reported to NCDENR. Spill_Amount _ 1000 gallons Impact.-- - -- Corrective Action: .- ..- - - None -detected. Ground area excess spillage was vacuumed. Ground site was treated with twenty-five pounds of lime. Forney Creek was tested twice daily for fecal coliform until June 04, 2014 when fecal coliform levels dropped to below NCDENR requirements for two (2) consecutive test results. Date: December 12, 1999 Time: 3:30 p.m. Location: ELSD Pump Station 98 9024 Unity Church Road Denver, North Carolina Incident: A dual pump failure and a dialer failure causing the pump station to overflow into a ditch line that emptied into Lake Norman. Spill Amount: Less than 500 gallons Impact: None detected. Corrective Action: Excess spillage was vacuumed and the ditch line was flushed with Approximately 1,000 gallons of water. Site was disinfected with five gallons of chlorine and twenty-five pounds of lime. Dialer alarm was replaced and reprogrammed with no further incidents. Spill Amount: Less than 500 gallons. Impact: None detected. Corrective Action: Excess spillage was vacuumed and the storm drain was flushed with approximately 1,000 gallons of water. Site was disinfected with ten gallons of chlorine and forty pounds of lime. Repairs were made with no further incidents at this location.
