On May 14, 2007, the 401 Oversight/Express Permitting Unit of the Division of Water Quality received a request
from Mr. Christopher Burkhardt of Froehling & Robertson, Inc., regarding a project known as "Mills Park
Elementary School (E22)" for acceptance into the Express Review Program. This letter advises you that your
project will be accepted into the Express Review Program once the following items are received:
1) The application fee of $1,000.00 [for 401 Water Quality Certification with major impacts (1/2 price)], made
payable to the North Carolina Division of Water Quality];
2) Five (5) complete and collated copies of the PCN Application (if it has been modified since the last
submission);
3) Five (5) copies of all new site plan information pertaining to this project (please refer to the Addendum
httn://h2o.enr.state.nc.us/ncwetlands/aupadd5 pdt);
4) Three (3) copies of the stormwater management plan (information not previously submitted) and all required
information (refer to http://h2o.enr.state.nc.us/ncwetlands/documents/Exu Rev SWMA pdfJ. Please
review the stormwater management requirements of the General Water Quality Certification that you are
requesting.
Thank you for your attention to this matter. You are responsible for contacting the US Army Corps of Engineers
to determine the applicable 404 Permit. If they determine that this project will require an Individual Permit, you
will not be withdrawn from the Express Review Program.
The clock for this project will not start until receipt of the completed application package and required fee are
received. If the impacts are greater than indicated on the Initial Interest Form and/or the services are different,
you may be required to remit an additional fee. If you have any questions or wish to discuss these matters further
