I am aware that there are significant penalties for submitting false information. 2-(L5 Da/2,( Signature of Official ariva, ,i,"-4_ iZ/i 4k Title NARRATIVE SUMMARY GENERAL PROGRAM INFORMATION: During the reporting period, the City received approval from NCDWR on January 10, 2020 for the updated Headworks Analysis that was submitted on September 24, 2019. Also, the City received approval from NCDWR on February 14, 2020 for the updated Enforcement Response Plan for the Dental Amalgam Option 1 which the POTW is implementing the 3 R's requirement. Eleven one time compliance reports were mailed to our dental discharges and Four were returned. The Program Elements dates listed on the Division's Database Program sheet are correct. Contact information has been updated to add the Interim City Manager and the WWTP ORC; these corrections are indicated on the Division's Database Program Sheet. GENERAL PERMIT INFORMATION: During the reporting period, the City modified one (1) IUP and removed (1) IUP from the City's Pretreatment Program. The City renewed 7 IUP's that became effective on July 1, 2020 and 1 IUP that became effective on September 1, 2020 Albaad, USA permit #0012 was modified to increase flow, due to increase in production. The City submitted this permit modification on March 19, 2020 and received approval from NCDWR on April 3, 2020 Commonwealth Brands permit #0001 was terminated by the City on January 13, 2020 due to the closure of this facility. The City submitted this permit termination on January 17, 2020 and received approval for removal from NCDWR on January 31, 2020 SIU INFORMATION: 1. Albaad, USA (IUP#0012) SIGNIFICANT NONCOMPLIANCE (SNC) INFORMATION: No SNC Events OTHER INFORMATION: During the first compliance period Albaad exceeded their Flow limit three times. A notice of violation was issued for each violation, in accordance with the ERP. These violations did not result in SNC. During the second compliance period, Albaad, USA experienced no violations of permit conditions or limitations. 2. Beta Fueling Systems, LLC. (IUP#0005) SIGNIFICANT NONCOMPLIANCE (SNC) INFORMATION: No SNC Events OTHER INFORMATION: During the first compliance period, Beta Fueling Systems LLC. violated permit conditions for failure to submit January's monthly report on time. This report was less than 30 days late and not considered to be SNC. A notice of violation was issued for this violation in accordance with the ERP. During both compliance periods, Beta Fueling System's experience no violations of permit limitations for pipe 01 or pipe 02. 3. Dorada Foods, LLC. Reidsville (IUP#0004) SIGNIFICANT NONCOMPLIANCE (SNC) INFORMATION: No SNC Events OTHER INFORMATION: During both compliance periods, Dorada Foods experience no violations of permit conditions or limitations. 4. DyStar (IUP#0007) SIGNIFICANT NONCOMPLIANCE (SNC) INFORMATION: No SNC Events OTHER INFORMATION: During the first compliance period, Dystar exceeded their flow limit once. A notice of violation was issued for this violation in accordance with the ERP. During the second compliance period, Dystar exceeded their flow limit once. A notice of violation was issued for this violation in accordance with the ERP. Dystar exceeded their pH limit twice; a notice of violation was issued for each violation, in accordance with the ERP. These violations did not result in SNC. 5. Envision Plastics/Consolidated Container (IUP#0011) SIGNIFICANT NONCOMPLIANCE (SNC) INFORMATION: No SNC Events OTHER INFORMATION: During the first compliance period, Envision Plastics experienced no violations of permit conditions or limitations. During the second compliance period, Envision Plastics exceeded their pH limit once; a notice of violation was issued in accordance with the ERP. This violation did not result in SNC. 6. Henniges Automotive (IUP#0010) SIGNIFICANT NONCOMPLIANCE (SNC) INFORMATION: No SNC Events OTHER INFORMATION: During both compliance periods, Henniges Automotive experienced no violations of permit conditions or limitations for pipe 01 or pipe 02. 7. Tamco (IUP#0008) SIGNIFICANT NONCOMPLIANCE (SNC) INFORMATION: No SNC Events OTHER INFORMATION: During both compliance periods, Tamco experience no violations of permit conditions or limitations. 8.
