REGAN Secretary LINDA CULPEPPER Interior Director On May 3, 2018 staff of the North Carolina Division of Water Resources (DWR) Winston-Salem Regional Office performed a compliance inspection of the subject wastewater irrigation system. This inspection was performed by DWR representative Jim Gonsiewski. The compliance inspection report for this inspection is enclosed for your review. A separate DWR office file review of self -monitoring data for the period of May 2017 through March 2018 was also conducted. The following items were noted in which permit conditions were not met: 1. Permit Condition IV.4 requires that effluent be sampled weekly for total residual chlorine. One chlorine sampling event was conducted in October 2017 and January 2018. No chlorine sampling was conducted in May 2017, June 2017, September 2017, November 2017, December 2017, February 2018, and March 2018. In the future, please ensure residual chlorine sampling is conducted weekly. It is our understanding that weekly residual chlorine sampling has been instituted by sampling directly under the effluent pipe after the chlorinator. 2. Permit Condition IV.4 limits the monthly effluent flow to a monthly average of 3500 gallons per day (gpd). This limit was exceeded in May 2017 (4506 gpd), September 2017 (4300 gpd), October 2017 (5016 gpd), November 2017 (4071 gpd), and February 2018 (4550 gpd). According to Steven Yarbrough, ORC for the facility, this excess flow was due to water line and valve leaks in the water system for the facility that have been repaired. Permit Condition IV.11(d) requires the Permittee to notify the NCDEQ Winston-Salem Regional Office (WSRO) if the facility is out of compliance with its permit limitations. No notifications were made to the WSRO concerning these exceedances. Be advised that if the requested information is not submitted to this office (as applicable) and the subject items reoccur, these violations may result in the issuance of a Notice of Intent to Enforce and are also subject to civil penalty assessment.
