The proposal is to construct a gabion wall along the Stewart stream bank at Graham
Street Transmission Structure 5 The work is proposed to start May 1 2012
Enclosed you will find five (5) copies of the completed PCN form and sketches of
proposed work to be completed for the above project The points of contact for this
project have been Alan Johnson and Michael Burkhard with the Division of Water
Quality Mooresville Regional office who are familiar with this project A field meeting
was held with Eric Shook with Duke Energy Mr Johnson Mr Burkhard Steve
Kichefski with US Army Corps of Engineers the design engineer and the contractor to
complete the work prior to developing the project plan
A check for the Minor Water Quality Application Fee in the amount of $240 00 is
enclosed

3. Grade 10' Haul Road to area of Gabion Wall construction;
4. Grade Foundation for Gabion Wall to bedrock of stream;
5. Install first course of gabion Wall (9 foot wide base);
6. Maintain pumping to dewater high turbity water in construction area
7. Complete installation of Gabion Baskets/Stone until final height is reached; remove
sandbag cofferdam as construction is completed;
8. Backfill area behind Gabions with Class I Rip Rap and properly compacted soil;
9. Perform Final Grading and Seed and Stabilize all disturbed areas.

1. All construction to be in accordance with Gabion Basket Manufacturers:' Specifications;
