REQUEST FOR MORE INFORMATION MODIFIED STORMWATER MANAGEMENT PLAN
Dear Mr Ward
On February 3 2011 the Division of Water Quality (DWQ) Issued a 401 Water Quality
Certification to Impact 0 35 acre of 404 Wetlands and 219 linear feet of perennial stream in
order to construct the Brightwater Medical Complex In Harnett County Condition 6 of this
certification requires that this project adhere to 401 Stormwater Management
Requirements
On March 26 2012 the DWQ received a stormwater management plan (dated March 23
2012) for the addition of the Brightwater Health Sciences Building on the Brightwater
Medical Complex campus The DWQ has reviewed the stormwater management plan and
has found it to be incomplete
Please provide the following information so that we may continue to review your project
Additional Information Requested
Please provide the Required Items C.hecklisT portion or the Wet Detention Basin
Supplement Form as well as any items from the form that were omitted Including the
planting plan for the vegetated shelf a signed and notarized Operation and
Maintenance Agreement the soils test documenting the level of the seasonal high
water table a construction sequence explaining how the wet detention pond will
be protected from sediment during the construction process etc

2 The sum of the volumes of the two forebays should comprise approximately 20
percent of the overall volume In the currently proposed design the forebays
together comprise approximately 40 percent of the overall volume
3 The elevations provided in the Supplement Form do not match the elevations shown
on Sheet C401 Please correct this error and re calculate the average depth It
appears that the average depth may be less than the minimum requirement of 3 0
feet
