Tom,
This is the Erosion Control Plan approval email that was sent to Linda by our plan review system:
Application Type: 04.02) Grading/Erosion Control Permit
Workflow: 04.02) Grading & Erosion Control
Project: Crosslands Phase II - SFD Erosion Control
A/P #: EN2000200
Acreage to be Disturbed: 1.20
https://winston-salem.idti2lanS._com/secure/pro'e ct/?prajectid=464022
Hello Linda Santos,
Thank you for submitting your project to City of Winston-Salem. Review of the plans for this project have
been completed. The design is adequate to meet the ordinance standards and the plans have been approved.
If you have not already done so, please provide an original signed and notarized Financial
Responsibility/Ownership form. This document is required prior to the issuance of the Grading/Erosion
Control Permit. If this has already been provided, then please disregard this statement.
If this project will disturb more than 1 acre or is part of a larger common plan of development, the
Grading/Erosion Control Permit cannot be issued until documentation is provided that coverage under the
NCGOI Construction Stormwater Permit (NPDES) has been obtained. Please obtain coverage under the
permit at the following website(https://edocs.deq.nc.gpv/Forms/NCGOI-NOI). Once coverage is obtained
please upload a copy to the project documents in the electronic plan review system. The Grading/Erosion
Control Permit will not be issued until proof of coverage under NCGO1 is provided.
Should you have any questions as you move forward please feel free to contact our offices. We will be happy
to assist you.

Tom,
The approval email from our plan review system has been adapted to have everything you need
to apply for the NCGOI Certificate of Coverage. That email went out to
linda@g]enwoodhomes.corn on 2/l/2021 at 11:28 am. Upload a PDF of that email for the
Approval Letter requirement in the e-NOI system.

Linda & Tom,
What you uploaded today is not what we need to complete the process on the Grading Permit.
You uploaded the NCG01 Plan Sheet documents which we already have in "Application
Submittal 005". Please follow the instructions on the remaining open issue outlined below. Let
me know if you have any questions.
"Since this project is proposed to disturb more than 1 acre or is part of a larger common plan of
development, the Grading/Erosion Control Permit cannot be issued until documentation is
provided that coverage under the NCGOI Construction Stormwater Permit (NPDES) has been
obtained. Please obtain coverage under the permit at the following website
(https:/Iedoes.deg.nc.gov/FormsINCGOI-NOI). Once coverage is obtained please upload a
copy to the project documents in the electronic plan review system. The Grading/Erosion
Control Permit will not be issued until proof of coverage under NCGO1 is provided."
