This permit shall be effective from the date ofissuance until July 31, 21 , and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate system for collecting d maintaining the required operational information will result in future compliance problems. Box 29535, Raleigh, North Carolina 27626-0535 Telephone 919-7 - 919-" - 91 n Equal opportunity AffVrnatve Action Employer 50% recycled/ to '* pest -consumer paper NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF ENVIRONMENT, HEALTH AND NATURAL RESOURCES I RECYCLE SYSTEM PERMIT In accordance with the provisions of Article 21 of Chapter 143, General Statutes of Now Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Waste Management of Carolinas, Incorporated Gaston County FOR THE construction and operation of a 3,900 GPD wastewater recycle system consisting of a 600 gallon oil/water separator with 15 gpm flow capacity, an oil skimmer, an ozone generator, a coalescing pack, a 65 gallon clarified water holding tank, a 65 gallon filtered water holding tank, a 30 gpm filter pac with 350 pounds of filter media, two (2) 200 ft2 20-micron cartridge filters and a 200 pound activated carbon filter to serve Waste Management of Carolinas, Inc.'s Gastonia Maintenance Facility, with no discharge of wastes to the surface waters, pursuant to the application received July 18, 1995, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Environment, Health and Natural Resources and considered a part of this permit. This permit shall be effective from the date of issuance until July 31, 2000, and shall be subject to the following specified conditions and limitations: I This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data, 3 The facilities shall be properly maintained and operated at all times. 4. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Environmental Management accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of the request will be considered on its merits and may or may not be approved. 5 Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with this perm, it, the approved plans and specifications, and other supporting materials. Mail the Certification to the Water Quality Permits and Engineering Unit, P.O. The facilities shall be effectively maintained and operated as a non -discharge, system to prevent the discharge of any wastewater resulting from the operation of this facility. 11. C. 27611 Dear Mr. Cleary: This is to reply to your letter of October 25, 1988, to Mr. Roger Lewis, Manager, Waste Management of N. C., Inc., in refference to Application WQ0000508, Truck Wash, Union Road, Gaston County and our telephone conversation on November 7, 1988. The items Nyou mention and our replies are the following: 1. The two soil borings were made within the proposed nitrification field. 2. Groundwater was not encountered in either of the it borings. 3. There are no indications of presence or absence of a seasonal high water table in that the land is a moderate "upland". 4. The existing well is to provide domestic use of water to the existing building at the site. No information is available on the well construction in that it has been existing for quite some time. It is estimated the well will pump 15 Gallons Per Minute, 5. There are no other wells within 500 feet of the proposed facility. Please advise if there are any questions on the above.
