Subject: Owner Affiliation and Facility Contacts for North Carolina NPDES Correspondence
Duke Energy NPDES Permitted Facilities
Dear Mr. Poupart:
As discussed with you by Richard Baker and Shannon Langley on July 5, 2017, Duke Energy has recently
noted an increased number of documents and correspondence from your agency being directed to the
attention of incorrect staff persons for our NPDES permitted facilities. A few recent examples include
the annual NPDES inspection report for the Mayo Steam Station and a Notice of Violation for the Cape
Fear plant being sent to the Vice President of Harris Nuclear Plant; as well as correspondence for the
Allen Steam Station sent to the South Carolina Director of Environmental Policy. In each.case; this has
led to delays in the permitted facility receiving information and impacts our ability to provide time
sensitive responses to the Department of Environmental Quality (DEQ).
Duke staff have discussed this situation with DEQ staff in the central and various regional offices. Based
on our conversations and' a review of information captured in your BIMS system, we believe the best
way to address. this is with the following included information:
1. A single name/ownership change form to request that the "owner affiliation" for entities Duke
Energy Progress, LLC (DEP) and Duke Energy Carolinas, LLC(DEC) be updated with my contact
information: There appear to be numerous obsolete contacts listed as "owner contact persons"
in your BIMS database- that field can be updated to include my name and the'othem removed.
2. The "permit billing contact person" should be updated for all. facilities to name Cynthia Winston,
Manager; 410 S. Wilmington Street, NCRH15, Raleigh NC 27601. She can be reached at
(919)546-5538 and cynthia.winston@duke-enerey.com.
3. The "facility contact person" for each facility should reflect the Station Manager at the site. A
list of current Station Managers for all DEP and DEC facilities is attached to this correspondence.
4. The fields for "permit contact person" and "persons with signatory authority" have been
recently updated as partof the a-dmr registration process and appear.accurate as of this time.
Based on this information, I would envision that general correspondence applicable to more than one
Duke Energy facility, such as groundwater information or other programmatic issues, should be directed

to my attention, while station specific communication should be directed to the Station Manager for the
site. Duke Energy will continue to routinely update the DEQ in a timely manner when facility contacts
change. If you think it would be helpful, we would be available to meet with your staff to discuss the
best way to assure accurate delivery of correspondence. We are providing copies of this letter to each
of the regional office supervisors responsible for NPDES permit oversight.

Comment: The general permit was reissued on August 1, 2012, and expires on July 31, 2015. A NOI to
continue_ permit coverage was submitted to the Division on January 26, 2015: which
included two additional outfalls (004 and 005) for junior generators installed at the facility.
The permittee has maintained permit coverage since September 10, 1993. The last
compliance evaluation inspection was conducted on September 1, 2009.

Proceed to the end of SR 1501 and turn left on SR 1500 (Lookout Dam Rd.). The Dam is located
at the end of SR 1500.

dischargers and/or drinking water supply intakes for a distance of several miles below the
existing point of discharge.

gpm of Bearing Cooling water); outfall 003 - 0.1008 MGD (20 gpm from the Lube Oil Coolers
and 50 gpm of Bearing Cooling water). These flows are based on the power generation units
running at maximum capacity.

PART IV - EVALUATION AND RECOMMENDATIONS
The applicant, Duke Energy Carolinas, LLC, operates a hydroelectric dam on the Catawba River
(constructed in 1915), which generates electric power used throughout Duke's service area. Complex
electrical generation machinery exists at this facility that requires cooling water during operation. This
cooling water is characterized as lube oil cooling water and bearing cooling water, which is used to
lubricate the turbine shaft during facility operation.
Pending a final review by the NPDES General Permits Unit, reissuance of a Certificate of
Coverage appears to be_ appropriate based on the existing wastewater sources.

been designated an authorized Representative of the applicant.
d) Final plans for the treatment system (if applicable). The plans must be signed and sealed by a North
Carolina registered Professional Engineer and stamped -"Final Design -Not released for construction".

Dear Ms. Allocco:
Duke Energy Carolinas, LLC hereby addresses inspection deficiencies denoted in
your letters of October 5, 2009. These letters summarized findings for Lookout
Shoals and Oxford hydroelectric facilities, which were inspected by your office on
September 1, 2009. The deficiencies cited were identical for both facilities. These
deficiencies and our responses are listed below:
"The facility ,is sampling oil and grease using a plastic container."
Response - No oil and grease samples were collected in a plastic container.
The plastic sampling appartatus, which is weighted, actually holds the -glass
bottle which is lowered into the water to obtain the sample. This sampling is
performed per 40 CFR 136.
"No downstream monitoring has been performed since 2.27.2007."
Response - Downstream sampling was terminated in March 2007 after a
conference call between Duke Energy and your office. Duke Energy was of
the understanding that downstream monitoring would not be required since
effluent monitoring was being performed. Subsequent'to these inspections,
downstream monitoring has been resumed at both hydroelectric facilities.
This downstream monitoring will continue on a quarterly basis.

Enclosed please find a copy of the Compliance Evaluation Inspection Report for the inspection conducted at the
subject facility on September 1, 2009, by Ms. Donna Hood of this Office.
At the time of the inspection, the permittee had ceased downstream sampling as of February 27, 2007.. Please
be advised that changes in sampling protocol require a written request for permit modification submitted to NPDES
Permitting in Raleigh. Until permit modification is requested and subsequent written permission received, the
sampling must continue as stated in the permit issued.
This report is being issued as a Notice of Deficiency (NOD) due to the failure to submit proper reporting
documentation for the wastewater treatment facility in violation of the subject NPDES Permit and North
Carolina General Statute (G.S.) 143-215.1, as detailed in the Laboratory and Upstream/Downstream
Samplinlz Sections of the attached report.

Dam Rd.). The Dam is located at the end of SR 1500.

The applicant, Duke Power Company, operates a hydroelectric dam on the Catawba River
(constructed in 1915), which generates hydroelectric power used throughout Duke's service area.
Complex electrical generation machinery exists at this facility that requires cooling water during
operation. Reissuance of a Certificate of Coverage appears to be appropriate based on the
existing wastewater sources.
Pending a final review by.the NPDES General Permits Unit, it is recommended that a
Certificate of Coverage be reissued as .requested.

(COC). This COC is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency,
dated May 9, 1994 (or as subsquently amended).

In accordance with your application for renewal of the subject Certificate of Coverage, the Division is forwarding
the enclosed General Permit. This renewal is valid until July 31, 2002. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated December 6,1983. If any parts, measurement
frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to request
an individual permit by submitting an individual permit application. Unless such demand is made, this certificate
of coverage shall be final and binding.
The Certificate of Coverage for your facility is not transferable except after notice to the Division. Use the enclosed
Permit Name/Ownership Change form to notify the Division if you sell or otherwise transfer ownership of the
subject facility. The Division may require modification or revocation and reissuance of the Certificate of Coverage.

PART IV - EVALUATION AND RECOMMENDATIONS
The applicant, Duke Power Company, operates a hydroelectric
dam on the Catawba River (constructed in 1919), which generates
hydroelectric power used throughout Duke's service area. Complex
electrical generation machinery exists at this facility that
requires cooling water during operation. Issuance of a General
Permit appears to be appropriate based on the existing wastewater
sources.
This facility actually straddles two counties (Catawba and
Iredell), however, it was decided to list this facility in
Iredell County since the access road originates from within this
county.
Pending receipt and approval of the WLA, it is recommended
that an NPDES Permit be issued.

report, plan or other document filed or required to be maintained under Article 21 or regulations of the Environmental
Management Commission implementing that Article, or .who falsifies, tampers with or knowingly renders inaccurate -
any recording or monitoring device or method required to be operated or maintained under Article 21 or "regulations of
the Environmental Management Commission implementing that Article, shall be guiltyof a misdemeanor
punishable by a fine not -to exceed $10,000, or by imprisonment not to exceed six months, or by both. (18 U.S.C.
Section 1001 provides a punishment by a fine of not more than $10,000 or imprisonment not more than 5 years, or
