A. Application:
1. Quantitative justification: Please provide a quantitative justification for the maximum dry tons to
be certified for each of the residuals generating source.
2. Samplingresults: esults: The TCLP, metals and Fecal results submitted are from over two years ago. The
sampling results need to be within a 12 month rolling period from the date of application. Please
provide an explanation to justify if the results were not sampled in the previous 12 months.
3. Tyson — AdvancedPierre Foods: The value of Nitrate -Nitrite -Nitrogen in the Residuals Source
Certification (RSC) form is 18, however the value 353 is used in the PAN calculations. The value
353 was incorrectly obtained as it is the sampling method and not the result. Please re -submit the
PAN calculations for the aforementioned facility.
B. Setback Waivers:
1. The submitted application maps for the following fields include reduced setbacks between residuals
land application fields and habitable residences or places of assembly under separate ownership.
These setback waivers must be submitted in accordance with 15A NCAC 02T .1108(e) in that they
shall be written, notarized, signed by all parties involved, and recorded with the county Register of
Deeds. A copy of a residuals setback waiver is attached.
2. If a setback waiver cannot be obtained for a field, the effected field must be buffered out so that it
is in compliance with all the 15A NCAC 02T .I 108(c) setbacks for habitable residences or places
of assembly under separate ownership. The new acreage for any reduced field shall be resubmitted
in an updated LASC 06-16 form
C. Compliance Boundaries:
The submittal did not include the rationale for the requested re -location of the compliance boundary
and these changes were not included in the maps. Please submit this information. Please note that
the re-established compliance boundary must be delineated or included in a footnote describing the
redefined compliance boundary in accordance with the June 9, 2011 memo "Map Guidance for
Residual Land Application Fields" and the January 31 , 2007 memo "Alternatives for Water Supply
Wells Inside the Compliance Boundary
D. En2ineerin2 Calculations:
1. Pursuant to 15A NCAC 02T .1104(a)(2): Please submit the buoyancy calculations for the storage
facility.
2. Pursuant to 15A NCAC 02T .1104(a): Please submit specifications describing materials to be
used, methods of construction, and means for ensuring quality and integrity of the finished
product, including leakage testing.
