On January 25 2012 the Division of Water Quality (DWQ) received your application dated January 20 2012 to fill
or otherwise impact 320 linear feet of intermittent stream channel (permanent impacts) 10 512 square feet of Jordan
Lake watershed protected riparian buffer 2 540 square feet of Jordan Lake watershed protected riparian buffer to
stabilize portions of a UT to Bolin Creek The DWQ has determined that your application was incomplete and/or
provided inaccurate information as discussed below The DWQ will require additional information in order to
process your application to impact protected streams and buffers on the subject property This project is on hold
and incomplete until we receive the additional information requested below If we do not receive the requested
information your project will be formally returned as incomplete Please provide the following information so that
we may continue to review your project

1 Section C 6f of the PCN application indicates that proposed Zone 1 impacts total 10 512 square
feet However plan sheet PL 1 indicates that the total impact to Zone 1 is 10 273 square feet (890 +
1 106 + 8 277) Please revise the PCN application and/or the plan sheet to reflect more accurate and
consistent information
2 Plan sheet PL 1 A portion of Zone 1 appears to have proposed impacts but this area is not shaded to
reflect an impact Please explain and/or revise plan sheet to reflect more accurate information
3 Please minimize the impact to the riparian buffer by moving the stock pile out of the riparian buffer

minimum of 260 stems per acre However DWQ requires riparian buffer planting at a density of 320
trees /acre surviving at the end of 3 years Please make adjustments to planting density
