Holly Sheltei Creek are class C Sw 303(d) waters of the State Holly Sheltei Creek is on the 303(4)
list for unpaired use for aquatic life due to biological integrity ciiteria NCDWQ is very concealed
with sediment and erosion impacts that could result fiom this pioject NCDWQ recommends that
the most piotective sediment and erosion control BMPs be implemented in accoiclance with Design
Standards in Sensitive Watersheds to reduce the risk of nutrient runoff to Holly Sheltei Cr eek
NCDWQ requests that toad design plans provide treatment of the stone water runoff through best
management practices as detailed in the most recent version of NCDWQ StorinWatei Best

As state in your e mail correspondence DWQ provided project specific comments to NCDOT foi
this proposed budge replacement in a letter dated August 31 2009 In addition to those comments 1
we also would also like to offer the following comments

Review of the project reveals the piesence of surface waters classified as C Sw HOW High Quality
Waters of the State in the pioject study area This is one of the highest classifications foi water
quality Pursuant to 1.)A NCAC 2H 1006 and 15A NCAC 2B 0224 NCDOT will be lequned to
obtain a State Stormwater Permit prior to construction except in North Carolina s twenty coastal
counties
Based on field observation widening the budge to West and lengthening to the South appeals to
mmirmize impacts and provide a greatei benefit to the system
BD 51035 Bridge 77 over South River Overflow, Sampson County
BD 5103T Bridge 78 over South River, Sampson County
Review of the project reveals the presence of surface waters classified as C Sw Outstanding
Resource Waters of the State in the project study area The water quality classification of C SIA
ORW is one of the highest classifications in the State NCDWQ is extremely concerned with any
impacts that may occur to streams with this classification It is prefer red that these resources be
avoided if at all possible If it is not possible to avoid these resources the impacts shall be
minimized to the greatest extent possible Given the potential for impacts to these iesources dui mg
the pioject implementation the NCDWQ requests that NCDOT strictly adhere to North Caiohna
regulations entitled Design Standards in Sensitive Watersheds (15A NCAC 04B 0124)
throughout design and construction of the project Pursuant to 15A NCAC 2H 1006 and 15A
NCAC 2B 0224 NCDOT will be requned to obtain a State Stormwater Permit prior to
construction except in North Carolina s twenty coastal counties
Based on field observation lengthening the Bridge 77 to the South and widening to tine West
appears to minimize impacts and provide a greater benefit to the system

General Comments Regarding Bi idge Replacement Projects
11 NCDWQ is very concerned with sediment and erosion impacts that could result fi om this pioject
NCDOT shall address these concerns by describing the potential impacts that may occur to the
aquatic environments and an) mitigating factors that would reduce the impacts

Activities
13 If a bridge is being replaced with a hydraulic conveyance other than another bridge NCDWQ
believes the use of a Nationwide Permit may be required Please contact the US Army Cotp of
Engineers to deter nine the required permits)
14 If the old bridge is iemoved no discharge of budge material into surface waters is allowed unless
otherwise authorized by the US ACOE Strict adherence to the Corps of Engineers guidelines foi
biidge demolition will be a condition of the 401 Water Quality Certification
1D Whenever possible NCDWQ piefers spanning structuies Spanning structures usually do not
require work v<rthin the stieam of giubbing of the stieambanks and do not iequiie stieam channel
realignment The hot izontal and vertical clearances piovided by biidges shall allow for human and
wildlife passage beneath the structure Fish passage and navigation by canoeists and boaters shall
not be blocked Bridge supports (bents) should not be placed in the stieam when possible

17 If concrete is used during construction a dry work area shall be maintained to pi event duect contact
between curing concrete and stieam water Water that inadvertently contacts uncut ed conci ete shall
not be discharged to surface waters due to the potential foi elevated pH and possible aquatic life and
fish kills
18 Iftempoiaty access ioads of detours ate constructed the site shall be graded to its preconstluctton
contours and elevations Disturbed areas shall be seeded of mulched to stabilize the soil and
appropriate native woody species shall be planted When using tempoiaty stluctuies the at ea shall
be cleat ed but not gt ebbed Cleat ing the at ea with chain saws mower s bush hogs of other
mechanized equipment and leaving the stumps and root mat intact allows the area to re vegetate
naturally and minimizes soil disturbance
19 Sediment and eiosion control measures sufficient to protect water iesouices must be implemented
and maintained in accordance with the most recent veiston of North Carolina Sediment and Erosion
Conti of Planning and Design Manual and the most recent vet sion of NCS000250
20 All work in of adjacent to stieam waters shall be conducted in a dry work area unless otherwise
approved by NCDWQ Approved BMP measures fiom the most culient version ofNCDOT
Constl action and Maintenance Activities manual such as sandbags i ock berms coffer dams and
othei diversion structuies shall be used to prevent excavation in flowing water
21 Heavy equipment shall be opei ated fiom the bank rather than in stream channels to order to
minimize sedimentation and reduce the likelihood of introducing other pollutants into streams This
equipment shall be inspected daily and maintained to prevent contamination of surface waters fiom
leaking fuels lubricants hydraulic fluids of other toxic mateuals
22 In most cases the NCDWQ piefeis the replacement of the existing structure at the same location
with i oad closes e If road closure is not feasible a tempos ary detours shall be designed and located
to avoid wetland impacts minimize the need for clearing and to avoid destabilizing stieam banks
If the structure will be on a new alignment the old structure shall be ieinoved and the approach fill
removed from the 100 year floodplam Approach fills shall be removed and restored to the natty al
ground elevation The area shall be stabilized with glass and planted with native tree species Tall
fescue shall not be used in riparian areas 'A
23 Any anticipated dewatering of access structures necessary foi construction of badges should be
addressed in the CE It is understood that final designs ale not deteimuned at the time the CE is
developed Howevei the CE should discuss the potential foi dewatering and access measuies
necessary due to bridge consti uction
24 Any impacts to waters of the state resulting fiom utility relocations associated with these budge
replacement piojects must be noted in the project document Utility relocations ale generally
authorized in accordance with the conditions of General 401 Certification Numbei 3819/Nationwide
Peimit No 12 for Utility Line Activities
