On December 8 2011 the Division of Water Quality (DWQ) received your application dated December 6 2011 to
fill or otherwise acre impact 0 068 acre of 404 wetlands 297 linear feet of perennial stream channel (permanent
impacts) 27 linear feet of intermittent stream channel (temporary impacts) 34 630 square feet of Zone 1 Neuse
River basin protected riparian buffer and 23 490 square feet of Zone 2 Neuse basin protected riparian buffer for a
residential development The DWQ has determined that your application and additional information was
incomplete and/or provided inaccurate information as discussed below The DWQ will require additional
information in order to process your application to impact protected streams and buffers on the subject property
This project is on hold and incomplete until we receive the additional information requested below If we do not
receive the requested information your project will be formally returned as incomplete Please provide the
following information so that we may continue to review your project

the PCN application proposes 27 linear feet of temporary stream impacts Please explain discrepancy and
revise and resubmit accurate and consistent information

Section C 61 is not an accurate statement Please revise and resubmit Page 7 of 12 of the PCN application
with more accurate information
4 This office does not believe that permanent impacts to the stream (Impact #3) as depicted on Sheet A were
quantified correctly The two stream segments that seem to be incorrect are directly adjacent to the
termini of the proposed impact where the existing stream is within the area of the proposed reconnection
(see attached map for clarification) Please explain why these segments would not be considered a
permanent impact
5 Please explain how additional impacts to the buffer are not warranted at the locations where rip rap on the
stream bank is proposed at both ends of the proposed stream connection Will a contractor be able to
place rip rap on the stream bank at these locations without impacting additional buffer beyond the
footprint of the rip rap?
6 Please explain how the near 90° turn and the perpendicular reconnection from the proposed pipe to the
stream at the termini of the proposed impact will not have adverse affects on the stream channel Please
provide rip rap specifications for the termini of the proposed pipe
7 Item E(2d) of the PCN form for this protect states The Stormwater Management plan has been provided
as an Appendix However the stormwater management plan was not provided Please provide a
complete stormwater management plan including a completed BMP Supplement Form for each proposed
BMP as well as all of the required items listed in the form Failure to provide a complete stormwater
management plan in the next submittal will result in a return of this application
Please submit five copies of this information within 30 calendar days of the date of this letter Please send
additional !information to DWQ Central Office, Attn Karen Higgins, 1650 Mail Service Center, Raleigh,
NC 27699 1650 If we do not receive this requested information within 30 calendar days of the date of this
letter your protect will be withdrawn and you will need to reapply with a new application and a new fee If you
need additional time to compile the information requested above please email Lauren Witherspoon
(lauren wtherspoon @ncdenr gov) requesting an extension and indicate a new date for submittal
This letter only addresses the application review and does not authorize any impacts to wetlands waters or
protected buffers Please be aware that any impacts requested within your application are not authorized (at this
