Dear Ms. Cook:
Please find attached a copy of Biocide Worksheet Form 101. This worksheet must be completed for each
additive, biocide, algaecide, and chemicals (Sodium Hydroxide, BL 171, BL 1541, BL 1347 & Caustic Soda) added to city
water prior to being pumped into the boiler unit to become boiler blowdown water at the Willis Hosiery Mill, Inc. facility in
Cabarrus County. The forms must be received by the Division of Water Quality no later than February 20, 2001. Submit
the forms to Mack Wiggins, Division of Water Quality, Stormwater and General Permits Unit, 1617 Mail Service Center,
Raleigh, NC 27699-1617.
If the Worksheet 101 forms are not received. by the Division of Water Qualityby February 20, 2001, your
application will be returned as incomplete. Discharging without a valid NPDES Permit to surface waters of the state, may
subject the discharger to a civil penalty of up to $10,000 per day.
