Dear Mr. Estridge:
The staff of the Fayetteville Regional Office has completed a review of the self -monitoring
report for the month of June 2004. This review revealed that the Town of Ansonville had
NPDES permit limit violations for Fecal Coliform and did not report such violations as called for
in your permit. Specifically.the Town is in violation of the NPDES permit for Part II. Section E
Reporting Requirements-6. Twenty-four Hour Reporting a. "The Permittee shall report to
the Director or the appropriate Regional. Office any noncompliance that potentially
threatens public health or the environment. Any information shall be provided orally
within 24 hours from the time the Permittee became aware of the circumstances."
We ask that you report all future violations as called for in your NPDES permit to the
Fayetteville Regional Office. Failure to report violations as called for in your NPDES permit
will be considered as noncompliance with the permit and may be subject to further action by this
office.
Please respond in writing to this office by October 15, 2004 addressing the cause of the fecal
coliform violations and what actions were taken to prevent the recurrence of similar situations.
Also address what if any action was taken utilizing your Sewer Use Ordinance to cover these
types of events. If youshould have any questions, please do not hesitate to contact Belinda S.

Subject: Response. to Notice of Violation
Dear Ms. Henson:
The following letter is in response to the Notice of Violation received by the Town of
Ansonville dated September 16, 2004. In the notice, two issues were discussed and both
required a written response from the Town.. ,The fecal violations for the month of June .and
the failure to meet the 24 hour reporting time will be addressed in this letter.
The. Ansonville WW.I.P failed to meet the required parameters for fecal coliform during the
weeks of June 17th and 24t. These violations were all caused by the same events. The
following is a summary of these events.
On June, 6, 2004, the Town of Ansonville began accepting wash -down -water from a local
trucking firm. The trucking firm, Howard Lisk Inc:, had contacted the Town prior to this
with a request to begin discharging this water to the Town's waste water collection system.
Before receiving any 'of this discharge, Daniel Wilson, ORC, visited the firrn's location and
evaluated the quality of the water to be discharged to the system. During this site visit, no
harmful products were found that should have affected the WWTP in any harmful way.
The water to be received was that used to wash earth products from trailers and water used
to wash tractor trailers. All of the water was first allowed to settle on their site to .remove
the majoiity of the solids. The Town Council was then notified by Daniel Wilson and felt
that this was an excellent means to generate revenue: The stipulations of the discharges
were that the Town would not receive any water in excess of 10,000 gallons per day and if
at any time the WWTP experienced an upset, the discharges would be stopped immediately
It should be noted that the average daily flow of .the WWTP is between 50,000 and 70,000
gallons per day.
On June 6, 2004, the first 5000 gallon load of this water was discharged to the collection
system. The plant immediately appeared to handle the ' increased flow :and no problems
were noted during the first two days. On June 8th, it was noticed that there was a decrease in
the settlabilty of the clarifiers at the WWTP and the rate of sludge wasting was increased.

The WWTP continued to receive this water at the volume of either 5000 or 10,000 gallons
per day each day with only minor changes being required to the treatment process. Then
on June 16th, the WWTP had a minor blower malfunction during the night and begin to
look slightly turbid the next day. Normally this type of malfunction would not result in
more than a day long problem. This was the beginning of the plant being upset. Although
the effluent appeared to be clear, the entire plant began to take on a deteriorating look: The
aeration basins began to grow darker by the day and the plant began to have an upset odor.
The blower run times were increased to try to offset the disturbances by the volume of
waste being took from the trucking firm. All of these problems were then traced back not
to the malfunctioning blower but to the amount of soap being discharged be the trucking
firm. On June 24th, Daniel Wilson contacted the trucking firm and notified them that they
must quit discharging any wash water to the system that contained the soap used to wash
the trucks.
The firm immediately stopped discharging any soap products to the system. On June 26th,
Daniel Wilson left for Honduras and was unable to be contacted by the Backup ORC,
Mandy Thomas. At the time of his departure, it appeared that the plant was beginning to
recover from the episodes of receiving the soap. On Monday, July 5a', 2004, Daniel Wilson
returned from being out of country and immediately recognized that. the W WI P was not
going to handle the load placed on it by the firm. Howard Lisk Inc was immediately
notified and no further discharge of any kind was received from their organization.
The violation associated with not notifying NCDENR within 24 hours of a failure to meet
our permitted parameters was an oversight on the part of the ORC, Daniel Wilson and
Belinda Henson have worked out a method by which Daniel will notify her via email
immediately upon receiving notification of failure to meet the parameters.

Dear_Mrs.. Springer:
This letter is in response to the recent laboratory inspection report received by the Town
of Ansonville. In replying to the recommendations listed on the letter the following
actions were taken:
1. The correct Chlorine residual standard was ordered from Hach and was received on
May 27th. I was out of town during that week, so that sample was put into use on the
following Tuesday, June 1, 2004.
2. There was also a comment concerning the need for an annual check of the pH meter's
temperature probe. This will be performed during the upcoming week and will be
available at your request.
Thank you for your assistance with these issues. Please contact the Town of Ansonville
with all additional comments and concerns.
